Title: Child nutritional status among births exceeding ideal family size in a high fertility population
Abstract

Ideal family size (IFS) is measured in social surveys to indicate unmet need for contraception and impending shifts in fertility behaviour. Whether exceeding IFS affects parental behaviour in ways that result in lower investments in child nutrition, well‐being, and educational attainment is not known. This study examines parental IFS and the association between exceeding stated ideals and child nutritional status in a high‐fertility, high‐mortality population in the Bolivian Amazon. Height‐for‐agez‐scores, weight‐for‐agez‐scores, weight‐for‐heightz‐scores, stunting, haemoglobin, and anaemia status in 638 children aged 0–5 years are predicted as a function of birth order in relation to parental IFS, adjusting for household characteristics and mother and child random effects. Children of birth orders above paternal IFS experience higher weight‐for‐agez‐scores when living further away from the market town of San Borja, consistent with underlying motivations for higher IFS and lower human capital investment in children in more remote areas (β = .009,p = .027). Overall, we find no statistical evidence that birth orders in excess of parental ideals are associated with compromised child nutrition below age 2, a period of intensive breastfeeding in this population. Despite a vulnerability to nutritional deficiencies postweaning for children age 2–5, there was no association between birth order in excess of parental ideals and lower nutritional status. Further studies examining this association at various stages of the fertility transition will elucidate whether reported ideal or optimal family sizes are flexible as trade‐offs between quality and quantity of children shift during the transition to lower fertility.

  1. Abstract Objective

    The epidemiologic link between cesarean birth and childhood obesity is unresolved, partly because most studies come from industrialized settings where many post‐birth factors affect the risk for obesity. We take advantage of an unusual ethnographic situation where hospital and cesarean birth modes have recently been introduced among Yucatec Maya subsistence farmers, but young children have had minimal exposure to the nutritional transition. While we expect to find very low rates of childhood obesity, we predict that cesarean‐born children will be larger and heavier than vaginally born children.

    Methods

    Weight and height were collected monthly on 108 children aged 0–5 (3576 observations total). Birth mode and birthweight were collected by maternal interview. Data were analyzed using linear mixed models that compare child growth [Maya population‐specificZ‐scores for weight‐for‐age and body mass index‐for‐age (WAZ and BMIZ)] in cesarean and vaginally born children aged 0–5 years.

    Results

    The cesarean rate was 20%, no children were obese, and 5% were overweight. Cesarean birth was a significant predictor of child WAZ and BMIZ after accounting for maternal effects, child birthweight, and sex. Children who were born by cesarean to mothers with high BMI had the highest WAZ of all children by 5 years of age, and the highest BMIZ of all children at all ages.

    Conclusion

    Cesarean‐born Maya children had higher BMI than vaginally born children, even in the absence of many known confounding factors that contribute to childhood obesity. Child growth was most sensitive to birth mode when mothers had high BMI.

  2. Abstract Objective

    Adrenarche, the biological event marked by rising production of dehydroepiandrosterone and its sulfate (DHEAS), may represent a sensitive period in child development, with important implications for adolescence and beyond. Nutritional status, particularly BMI and/or adiposity, has long been hypothesized as a factor in DHEAS production but findings are inconsistent, and few studies have examined this among non‐industrialized societies. In addition, cortisol has not been included in these models. We here evaluate effects of height‐ (HAZ), weight‐ (WAZ), and BMI‐ (BMIZ) for‐age on DHEAS concentrations among Sidama agropastoralist, Ngandu horticulturalist, and Aka hunter‐gatherer children.

    Methods

    Heights and weights were collected from 206 children aged 2–18 years old. HAZ, WAZ, and BMIZ were calculated using CDC standards. DHEAS and cortisol assays were used to determine biomarker concentrations in hair. Generalized linear modeling was used to examine effects of nutritional status on DHEAS concentrations, as well as cortisol, controlling for age, sex, and population.

    Results

    Despite the prevalence of low HAZ and WAZ scores, the majority (77%) of children had BMIz‐scores >−2.0 SD. Nutritional status has no significant effect on DHEAS concentrations, controlling for age, sex, and population. Cortisol, however, is a significant predictor of DHEAS concentrations.

    Conclusions

    Our findings do not support a relationship between nutritional status and DHEAS. Instead, results suggest an important role for stress and ecology in DHEAS concentrations across childhood. Specifically, effects of environment via cortisol may be influential to patterning of DHEAS. Future work should investigate local ecological stressors and their relationship to adrenarche.

  3. Abstract Background

    Ethnographic work among high altitude populations has shown that children are highly mobile—the most recent expression of this is the educational migration of children born at high altitude to boarding schools at lower altitudes. The impact of these patterns of migration on size for age are unknown.

    Aim

    We investigated the association between growth in weight and height and educational migration in ethnic Tibetan children living in and out of their natal communities.

    Subjects and methods

    Five hundred and fifty eight children ages three to sixteen from the Nubri Valley, Nepal participated in this study. Three hundred children were living in natal villages and 258 were attending boarding schools in Kathmandu. Height, weight, and skinfold thicknesses were collected and matched to demographic data from the community.

    Results

    There was no association between altitude of family residence and size for age z‐scores. Males had lower z‐scores than females; z‐scores for both groups declined with age. Differences in size for age among children in boarding schools were associated with two factors: sex and type of boarding school (individual sponsor or group funded). Individuals attending individually sponsored schools had greater size for age compared to children in group funded schools or in their natal villages; younger children in collectively funded schools were smaller than village peers.

    Conclusions

    Despite popular perceptions, educational outmigration in Himalayan communities may not be associated with improved child growth outcomes and investment in community level schools may be a practical solution for improving child growth and physical and mental health.

  4. Abstract Objectives

    Despite repeated governmental and nongovernmental interventions, anemia remains a widespread public health concern in Peru. This article examines nutritional and disease factors associated with the prevalence of anemia and response to iron supplementation in Peruvian children, with the aim of understanding the lack of success of national programs.

    Methods

    Data come from children, aged 2‐5 years (n = 102), living in peri‐urban Lima. Predictors of anemia and response to iron supplementation were explored at the individual, maternal, household, and environmental levels using logistic regression models, adjusted for clustering by household.

    Results

    Half of the children in this sample were anemic and 50% of the anemic children responded to iron supplementation. We identified several factors that were associated with child anemia status and response to iron supplementation. Lower weight‐for‐age z‐scores and the winter season were strong predictors of child anemia status and nonresponse to iron supplementation. Living with paternal grandparents was protective against anemia and elevated C‐reactive protein at the time of the final interview was associated with a lack of response to iron supplementation.

    Conclusions

    The findings of this study document an association between children's anemia status and their nutritional and ecological environment, highlighting the importance of examining anemia within a specific context to better understand the factors driving this important health problem.

  5. Abstract Introduction

    Cesarean delivery is associated with childhood obesity in urbanized and sanitized environments, but this is not well‐studied across broader epidemiologic settings. We examine birth mode, household conditions, and child growth in a Yucatec Maya farming community with limited sanitary infrastructure. We test the hypotheses that household infrastructure and epidemiologic conditions: (1)modulateassociations between cesarean delivery and growth, and (2) are more strongly associated with growth compared to birth mode.

    Methods

    Birth mode, household characteristics (construction materials/waste disposal/water variables) and growth were examined in 106 children (0–5 years, 3554 longitudinal observations). Ahousehold modification index(HMI) was created to indicate “baseline” (more traditional) versus “modified” (more modernized) houses. Weight/height/BMI Z‐scores (WHO‐standards) were modeled with respect to HMI and birth mode using nested generalized linear mixed‐effects models. A series of interaction models were then compared.

    Results

    Four household modification variables (concrete roof and floor, latrine/septic tank, burning trash) were associated with greater growth gains and used to compose the HMI. Cesarean delivery was associated with slightly higher Z‐scores for most anthropometric outcomes. HMI modulated the association between birth mode and children's weight and height, but not BMI, trajectories. Weight and BMI Z‐scores, but not height Z‐scores, were higher at all ages under high HMI (“modified”) versus low HMI (“baseline”) conditions regardless of birth mode.

    Conclusions

    Understudied household conditions can modulate the association between cesarean delivery and childhood growth. These findings contribute to a large body of literature linking cesarean delivery to a variety of child health outcomes including obesity.
