Title: Regional variations in longitudinal pulmonary function: A comparison of Hispanic and non‐Hispanic subjects with cystic fibrosis in the United States
Abstract Background

Hispanic subjects with cystic fibrosis (CF) have increased morbidity and mortality than non‐Hispanic white subjects. The ethnic disparity in mortality varies by region. Factors influencing pulmonary function vary by both ethnicity and region.

Objective

To determine if the ethnic difference in pulmonary function varies by region.

Methods

This retrospective cohort study compared differences in longitudinal pulmonary function (percent‐predicted forced vital capacity [FVC], forced expiratory volume in 1 second [FEV1], forced expiratory flow at 25% to 75% [FEF25‐75], FEV1/FVC, and FEV1decline) between Hispanic and non‐Hispanic white subjects with CF by Census region of the United States (West, South, Midwest, and Northeast). Subjects were of ages 6 to 25 years and in the CF Foundation Patient Registry from 2008 to 2013. We used linear mixed effects models with subject‐specific slopes and intercepts, adjusting for 14 demographic and clinical variables.

Results

Of 14 932 subjects, 1433 (9.6%) were Hispanic and 13 499 (90.4%) were non‐Hispanic white. Hispanic subjects’ FEV1was 9.0% (8.3%‐9.8%) lower than non‐Hispanic white subjects in the West, while Hispanic subjects’ FEV1was only 4.0% (3.0%‐5.0%) lower in the Midwest, 4.4% (3.1%‐5.7%) lower in the Northeast, and 4.4% (3.2%‐5.5%) lower in the South. Similarly, FVC and FEF25‐75were lower among Hispanic subjects compared to non‐Hispanic white subjects in all US regions, with the biggest differences in the West. Only in the West was FEV1/FVC significantly lower in Hispanic subjects (−0.019; −0.022 to −0.015). FEV1decline was not significantly different between ethnicities in any region.

Conclusions

In CF, Hispanic subjects have lower pulmonary function than non‐Hispanic white subjects in all geographic regions with the largest difference in occurring in the West.

  1. Background

    Mucociliary clearance is a main defense mechanism of the airway and is impaired in ciliary dyskinesia. The objective of this study was to evaluate the prevalence of chronic rhinosinusitis (CRS) and its characteristics in bronchiectasis patients suspected of harboring ciliary dyskinesia.

    Methods

    Bronchiectasis patients referred to a rhinology clinic for nasal brush biopsy (NBB) were included in this study. NBB was performed using a curettage technique whereby ciliated epithelial cells were obtained from the surface of the inferior nasal turbinate. Results of transmission electron microscopy findings, primary ciliary dyskinesia (PCD) gene (35 genes) analyses (Invitae), and sinus computed tomography (CT) scans were reviewed.

    Results

    Twenty‐three patients (age, 54 ± 2.9 years) were referred for NBB between 2015 and 2018. Thirteen patients (56.5%) met the criteria for diagnosis of CRS. Nineteen patients had ciliary ultrastructural defects. The most common finding was compound cilia (n = 11, 47.8%). Five patients (21.7%) had central microtubule defects (CMD) with higher forced expiratory volume in 1 second (FEV1) at the time of referral than those without CMD (CMD+, 91 ± 3.7%; CMD, 73.5 ± 5.7%;p= 0.023). Of 15 subjects with a PCD gene panel, 67% (9 of 15) carried at least 1 gene associated with PCD. Only 1 patient reached diagnosis of PCD. Approximately 50% of non‐PCD carriers had a smoking history (p< 0.05). Lund‐Mackay scores did not significantly differ between PCD and non‐PCD carriers (p= 0.72).

    Conclusion

    Nearly half of bronchiectasis patients referred for NBB had concurrent CRS. The presence of ciliary abnormalities was not amplified in bronchiectasis patients with CRS compared to those without CRS. Extrinsic factors may be related to ciliary structural abnormalities in non‐PCD gene carriers.

  2. Abstract

    Cystic fibrosis (CF) is characterized by chronic respiratory infections which progressively decrease lung function over time. Affected individuals experience episodes of intensified respiratory symptoms called pulmonary exacerbations (PEx), which in turn accelerate pulmonary function decline and decrease survival rate. An overarching challenge is that there is no standard classification for PEx, which results in treatments that are heterogeneous. Improving PEx classification and management is a significant research priority for people with CF. Previous studies have shown volatile organic compounds (VOCs) in exhaled breath can be used as biomarkers because they are products of metabolic pathways dysregulated by different diseases. To provide insights on PEx classification and other CF clinical factors, exhaled breath samples were collected from 18 subjects with CF, with some experiencing PEx and others serving as a baseline. Exhaled breath was collected in Tedlar bags during tidal breathing and cryotransferred to headspace vials for VOC analysis by solid phase microextraction coupled to gas chromatography–mass spectrometry. Statistical significance testing between quantitative and categorical clinical variables displayed percent-predicted forced expiratory volume in one second (FEV1pp) was decreased in subjects experiencing PEx. VOCs correlating with other clinical variables (body mass index, age, use of highly effective modulator treatment (HEMT), and the need for inhaled tobramycin) were also explored. Two volatile aldehydes (octanal and nonanal) were upregulated in patients not taking the HEMT. VOCs correlating to potential confounding variables were removed and then analyzed by regression for significant correlations with FEV1pp measurements. Interestingly, the VOC with the highest correlation with FEV1pp (3,7-dimethyldecane) also gave the lowestp-value when comparing subjects at baseline and during PEx. Other VOCs that were differentially expressed due to PEx that were identified in this study include durene, 2,4,4-trimethyl-1,3-pentanediol 1-isobutyrate and 5-methyltridecane. Receiver operator characteristic curves were developed and showed 3,7-dimethyldecane had higher ability to classify PEx (area under the curve (AUC) = 0.91) relative to FEV1pp values at collection (AUC = 0.83). However, normalized ΔFEV1pp values had the highest capability to distinguish PEx (AUC = 0.93). These results show that VOCs in exhaled breath may be a rich source of biomarkers for various clinical traits of CF, including PEx, that should be explored in larger sample cohorts and validation studies.

  3. Abstract Background

    Retaining women and racially minoritized individuals in engineering programs has been a subject of widespread discussion and investigation. While the sense of belonging and its link to retention have been studied based on student characteristics, there is an absence of studies investigating the importance of students' social identities to their sense of belonging in engineering.

    Purpose/Hypothesis

    This study examines differences in race/ethnic identity centrality, gender identity centrality, and sense of belonging in engineering by subgroups of undergraduate engineering students at Hispanic‐Serving Institutions (HSIs). Subsequently, it examines the extent to which these identity centralities predict a sense of belonging in engineering for each subgroup.

    Design/Method

    Survey data was collected from 903 Latinx and 452 White undergraduate engineering students from seven HSIs across the continental United States. Multivariate analysis of variance and sequential multivariate linear regression were used to evaluate the research questions.

    Results

    Latinx students had higher identity centralities but a similar sense of belonging in the engineering community as White students. Latinos and Latinas had an equivalent sense of belonging in engineering, whereas White women were higher than White men. In the full models, race/ethnic identity centrality significantly, and positively predicted a sense of belonging in engineering for Latinos and White women. Gender identity centrality was not a significant predictor of a sense of belonging in engineering for either Latinx or White students.

    Conclusions

    Race/ethnic and gender identity centrality are differentially important to the sense of belonging in engineering for students at Hispanic‐Serving Institutions based on their group membership at the intersection of race and gender.

  4. Abstract Introduction

    Certain subpopulations in the United States are highly vulnerable to tobacco initiation and addiction, and elimination of disparities among those groups is crucial to reducing the burden of tobacco use.

    Aims and Methods

    This study evaluated the racial and ethnic differences in smoking initiation of menthol flavored cigarettes and cigars among never-users, and in subsequent tobacco use among new users of menthol-flavored products, using longitudinal data from waves 1–4 of the Population Assessment of Tobacco and Health Study. The outcomes of interest were new use of menthol-flavored products, and subsequent past 30-day and past 12-month cigarette and cigar smoking, irrespective of flavors, after initiation.

    Results

    The percentages of new users of menthol-flavored cigarettes and cigars at waves 2–4 were disproportionately higher in non-Hispanic black and Hispanic than in non-Hispanic white people. Adjusting for age and sex, black people who first used any menthol cigars had higher risk of past 30-day use of the same cigar category at the subsequent wave (adjusted risk ratio, aRR 1.48; 95% confidence interval [CI] 1.11 to 1.96) and past 12 months (aRR 1.74; 95% CI 1.55 to 2.63) compared to non-Hispanic white smokers. Black people who first used menthol-flavored cigarettes had marginally higher risk of subsequent past 30-day cigarette use (aRR 1.44; 95% CI 0.99 to 2.10) compared with their non-Hispanic white counterparts.

    Conclusions

    This study shows that racial and ethnic differences exist in both initiation of menthol-flavored tobacco products and product-specific subsequent use after first using menthol-flavored products; black and Hispanic people have higher rates of initiation; black people also have higher rates of subsequent use.

    Implications

    Use of menthol flavors in tobacco products is confirmed to be a contributor to large disparities in tobacco use; black and Hispanic people are more likely to maintain smoking through use of mentholated products than non-Hispanic white people. The findings suggest educational and regulatory actions on menthol-flavored tobacco products including restricting the selective marketing to vulnerable communities and banning characterizing flavors in cigarettes and cigars may reduce tobacco-related disparities and inform the Food And Drug Administration’s evidence-based rulemaking process.

This research paper investigates the relationship between race/ ethnicity, gender, first-generation college student status, and engineering identity using cross-sectional data from early-career engineering majors. Measures of engineering identity are increasingly used in models of engineering education to evaluate how identity contributes to success and persistence of engineering students. Engineering identity is generally assumed to contribute to educational success, with stronger engineering identity leading to persistence. At the same time, data clearly shows that persistence of engineering students varies by race/ethnicity and gender. Given these previous findings, we would expect to find that engineering identity will vary by race/ ethnicity, gender, and first generation status. Yet, relatively little work has quantitatively compared how engineering identity differs across racial/ ethnic groups and gender. While researchers are increasingly trying to gain a better understanding of engineering identity among Latina students, for example, the literature has not yet adequately accounted for how Latina students differ from their non-Hispanic white peers. This works seeks to address that gap in the literature with an exploration of the ways that race/ethnicity, gender, and first generation status work together to impact engineering identity among early-career engineering students at four public Hispanic-Serving Institutions (HSIs) in the Southwestern United States. We conducted surveys as part of a longitudinal study on STEM education. Data discussed here comes from baseline surveys of three cohorts of engineering students (N=475). Approximately two-thirds of the respondents were attending a traditional 4-year university while the remainder (N=159) were attending community college at the time of the survey. Approximately two-thirds of the respondents identified as Latinx, 27% identified as female, and 26.5% reported that they were first-generation college students. While expectations were that engineering identity would vary by race/ethnicity and gender, preliminary analyses of our data unexpectedly reveal no significant differences between Latinx and White students in terms of their engineering identity and no significant differences in engineering identity between male and female students. Interactions between race/ethnicity and gender were also tested and yielded no significant differences between early-career Latinx and White students in terms of their engineering identity. Finally, students who reported that they will be the first in their family to get a college degree had significantly lower engineering identity scores (=-.422; p=.001). These results lead us to conclude that first generation status at HSIs may be more important than gender and race/ ethnicity in the development of engineering identity for early career students.
