Title: Testing reciprocal associations between child anxiety and parenting across early interventions for inhibited preschoolers
Background

Given the robust evidence base for the efficacy of evidence‐based treatments targeting youth anxiety, researchers have advanced beyond efficacy outcome analysis to identifymechanismsof change and treatment directionality. Grounded in developmental transactional models, interventions for young children at risk for anxiety by virtue of behaviorally inhibited temperament often target parenting and child factors implicated in the early emergence and maintenance of anxiety. In particular, overcontrolling parenting moderates risk for anxiety among highly inhibited children, just as child inhibition has been shown to elicit overcontrolling parenting. Although longitudinal research has elucidated the temporal unfolding of factors that interact to place inhibited children at risk for anxiety, reciprocal transactions between these child and parent factors in the context of early interventions remain unknown.

Method

This study addresses these gaps by examining mechanisms of change and treatment directionality (i.e., parent‐to‐child vs. child‐to‐parent influences) within a randomized controlled trial comparing two interventions for inhibited preschoolers (N = 151): the multicomponent Turtle Program (‘Turtle’) and the parent‐only Cool Little Kids program (‘CLK’). Reciprocal relations between parent‐reported child anxiety, observed parenting, and parent‐reported accommodation of child anxiety were examined across four timepoints: pre‐, mid‐, and post‐treatment, and one‐year follow‐up (NCT02308826).

Results

Hypotheses were tested via latent curve models with structured residuals (LCM‐SR) and latent change score (LCS) models. LCM‐SR results were consistent with the child‐to‐parent influences found in previous research on cognitive behavioral therapy (CBT) for older anxious youth, but only emerged in Turtle. LCS analyses revealed bidirectional effects ofchangesin parent accommodation and child anxiety during and after intervention, but only in Turtle.

Conclusion

Our findings coincide with developmental transactional models, suggesting that the development of child anxiety may result from child‐to‐parent influences rather than the reverse, and highlight the importance of targeting parentandchild factors simultaneously in early interventions for young, inhibited children.

  1. Background

    Callous‐unemotional (CU) traits are associated with chronic and escalating trajectories of antisocial behavior. Extant etiologic studies suggest that heritability estimates for CU traits vary substantially, while also pointing to an environmental association between parenting and CU traits.

    Methods

    We used twin modeling to estimate additive genetic (A), shared environmental (C), and nonshared environmental (E) influences on CU traits, measured with the Inventory of Callous‐Unemotional Traits (ICU) and its subscales. Our sample included 600 twin pairs (age 6–11, 230 monozygotic) from neighborhoods with above‐average levels of family poverty, a risk factor for antisocial behavior. We examined the extent to which correlations between parenting, measured via parent and child report on the Parental Environment Questionnaire, and CU traits reflected genetic versus environmental factors. Then, we tested whether parenting moderated the heritability of CU traits.

    Results

    In the context of lower‐income neighborhoods, CU traits were moderately to highly heritable (A = 54%) with similar moderate‐to‐high nonshared environmental influences (E = 46%). Bivariate models revealed that associations between CU traits andwarmparenting were genetic (rA = .22) and environmental (rE = .19) in origin, whereas associations between CU traits andharshparenting were largely genetic in origin (rA = .70). The heritability of CU traits decreased with increasing parental warmth and decreasing harshness.

    Conclusions

    Callous‐unemotional traits are both genetic and environmental in origin during middle childhood, but genetic influences are moderated by parenting quality. Parenting may be an important target for interventions, particularly among youth with greater genetic risk.

  2. Background

    Early‐onset (3–8 years old) disruptive behavior disorders (DBDs) have been linked to a range of psychosocial sequelae in adolescence and beyond, including delinquency, depression, and substance use. Given that low‐income families are overrepresented in statistics on early‐onset DBDs, prevention and early‐intervention targeting this population is a public health imperative. The efficacy of Behavioral Parent Training (BPT) programs such as Helping the Noncompliant Child (HNC) has been called robust; however, given the additional societal and structural barriers faced by low‐income families, family engagement and retention barriers can cause effects to wane with time. This study extends preliminary work by examining the potential for a Technology‐Enhanced HNC (TE‐HNC) program to improve and sustain parent skill proficiency and child outcomes among low‐income families.

    Methods

    A randomized controlled trial with two parallel arms was the design for this study. A total of 101 children (3–8‐years‐old) with clinically significant problem behaviors from low‐income households were randomized to HNC (n = 54) or TE‐HNC (n = 47). Participants were assessed at pre‐treatment, post‐treatment, 3‐month, and 6‐month follow‐ups. Primary outcomes were parent‐reported and observed child behavior problems. Secondary outcomes included observed parenting skills use (ClinicalTrials.gov Identifier: NCT02191956).

    Results

    Primary analyses used latent curve modeling to examine treatment differences in the trajectory of change during treatment, maintenance of treatment gains, and levels of outcomes at the 6‐month follow‐up. Both programs yielded improvements in parenting skills and child problems at post‐treatment. However, TE‐HNC families evidenced greater maintenance of parent‐reported and observed child behavior and observed positive parenting skills at the 6‐month follow‐up.

    Conclusions

    Our findings contribute to an ongoing line of work suggesting that technology‐enhanced treatment models hold promise for increasing markers of engagement in BPT and sustaining long‐term outcomes among low‐income families.

  3. Abstract Objective

    Collisions between bicycles and motor vehicles are one of the leading risk factors for injury and death in childhood and adolescence. We examined longitudinal and concurrent effortful control (EC) as predictors of risky bicycling behavior in early- to mid-adolescence, with age and gender as moderators. We also examined whether EC was associated with parent-reported real-world bicycling behavior and all lifetime unintentional injuries.

    Methods

    Parent-reported EC measures were collected when children (N = 85) were 4 years old and when they were either 10 years (N = 42) or 15 years (N = 43) old. We assessed risky bicycling behavior by asking the adolescents to bicycle across roads with high-density traffic in an immersive virtual environment. Parents also reported on children’s real-world bicycling behavior and lifetime unintentional injuries at the time of the bicycling session.

    Results

    We found that both longitudinal and concurrent EC predicted adolescents’ gap choices, though these effects were moderated by age and gender. Lower parent-reported early EC in younger and older girls predicted a greater willingness to take tight gaps (3.5 s). Lower parent-reported concurrent EC in older boys predicted a greater willingness to take gaps of any size. Children lower in early EC started bicycling earlier and were rated as less cautious bicyclists as adolescents. Adolescents lower in concurrent EC were also rated as less cautious bicyclists and had experienced more lifetime unintentional injuries requiring medical attention.

    Conclusion

    Early measures of child temperament may help to identify at-risk populations who may benefit from parent-based interventions.

  4. Lay Summary

    This study examined parenting stress across time among parents of young children with ASD concerns before receiving a diagnosis. Parents of children with ASD concerns reported consistently higher levels of parenting stress compared to parents of children with other developmental concerns and parents of children with no concerns. Also, ASD concerns predicted parenting stress in addition to other parent and child predictors of parenting stress. These findings highlight the need to better support families before an ASD diagnosis.

Despite a recent surge in research examining parent–child neural similarity using fMRI, there remains a need for further investigation into how such similarity may play a role in children's emotional adjustment. Moreover, no prior studies explored the potential contextual factors that may moderate the link between parent–child neural similarity and children's developmental outcomes. In this study, 32 parent–youth dyads (parents:Mage= 43.53 years, 72% female; children:Mage= 11.69 years, 41% female) watched an emotion-evoking animated film while being scanned using fMRI. We first quantified how similarly emotion network interacts with other brain regions in responding to the emotion-evoking film between parents and their children. We then examined how such parent–child neural similarity is associated with children's emotional adjustment, with attention to the moderating role of family cohesion. Results revealed that higher parent–child similarity in functional connectivity pattern during movie viewing was associated with better emotional adjustment, including less negative affect, lower anxiety, and greater ego resilience in youth. Moreover, such associations were significant only among families with higher cohesion, but not among families with lower cohesion. The findings advance our understanding of the neural mechanisms underlying how children thrive by being in sync and attuned with their parents, and provide novel empirical evidence that the effects of parent–child concordance at the neural level on children's development are contextually dependent.

    SIGNIFICANCE STATEMENTWhat neural processes underlie the attunement between children and their parents that helps children thrive? Using a naturalistic movie-watching fMRI paradigm, we find that greater parent–child similarity in how emotion network interacts with other brain regions during movie viewing is associated with youth's better emotional adjustment including less negative affect, lower anxiety, and greater ego resilience. Interestingly, these associations are only significant among families with higher cohesion, but not among those with lower cohesion. Our findings provide novel evidence that parent–child shared neural processes to emotional situations can confer benefits to children, and underscore the importance of considering specific family contexts in which parent–child neural similarity may be beneficial or detrimental to children's development, highlighting a crucial direction for future research.
