The mosquito-borne disease (malaria) imposes significant challenges on human health, healthcare systems, and economic growth/productivity in many countries. This study develops and analyzes a model to understand the interplay between malaria dynamics, economic growth, and transient events. It uncovers varied effects of malaria and economic parameters on model outcomes, highlighting the interdependence of the reproduction number () on both malaria and economic factors, and a reciprocal relationship where malaria diminishes economic productivity, while higher economic output is associated with reduced malaria prevalence. This emphasizes the intricate interplay between malaria dynamics and socio-economic factors. The study offers insights into malaria control and underscores the significance of optimizing external aid allocation, especially favoring an even distribution strategy, with the most significant reduction observed in an equal monthly distribution strategy compared to longer distribution intervals. Furthermore, the study shows that controlling malaria in high mosquito biting areas with limited aid, low technology, inadequate treatment, or low economic investment is challenging. The model exhibits a backward bifurcation implying that sustainability of control and mitigation measures is essential even when is slightly less than one. Additionally, there is a parameter regime for which long transients are feasible. Long transients are critical for predicting the behavior of dynamic systems and identifying factors influencing transitions; they reveal reservoirs of infection, vital for disease control. Policy recommendations for effective malaria control from the study include prioritizing sustained control measures, optimizing external aid allocation, and reducing mosquito biting.

Puddles, creeks, and drainage: connected histories of water and malaria in Zanzibar, 1910–2021
Abstract This article draws on a combination of archival and oral sources in order to highlight the connected histories of water and malaria on the Indian Ocean island of Zanzibar (Unguja) over the past century. We argue that in both the past and the present, water and malaria have been deeply connected, through the lifecycle of the disease, through control measures enacted, and in how Zanzibari residents continue to talk about the relationship between rain, puddles, stagnant water, and disease. Archival sources show that between roughly 1913 and 1957, the British colonial government managed malaria by managing water. That entailed a focus on watery breeding sites that included attention to both the micro: cattle hoofprints, and the macro: a multi-decade drainage of the tidal creek and surrounding swamp. These water-based malaria control efforts were limited in their efficacy by being geographically restricted to urban Zanzibar Town, and often limited to just the Stone Town area. 98 interviews conducted with Zanzibaris about malaria indicate that people continue to draw strong connections between malaria and the environment (mazingira), and that they most commonly discuss water’s relationship with malaria in terms of puddles (dimbwi/madimbwi), small puddles (kidimbwi/vidimbwi) and stagnant water (maji yaliyotuama).

Abstract BackgroundAs Namibia attempts to eradicate locally transmitted cases of malaria, epidemiological strategies, interventions, and outreach require a sound understanding of indigenous knowledge and practice. Research describing local explanatory models of disease can be of value in these efforts by elucidating how disease is interpreted and treated. To understand how perceptions of malaria infection and treatment may influence health-seeking behaviour, cultural models of the disease were explored in two ethnic groups in rural northwest Namibia. MethodsMixed-sex focus groups of 4–8 individuals were conducted in the Kunene region of Namibia. All participants were either Himba or Herero and lived between 14 and 57 km of the regional town centre of Opuwo. Discussion prompts were designed to assess knowledge, beliefs, and norms about malaria, including causes, symptoms, treatment, and prevention. ResultsFocus groups reported universal difficulty in discrimination between malaria and respiratory infections, the former of which was often only diagnosed at the hospital. Some recognized mosquitoes as the source of malaria, particularly the more formally educated Herero, but all also reported other causes. Notably these causes, including dietary and temperature-based origins, were considered unavoidable. Himba and Herero believed that malaria was infectious person-to-person and incorrectly believed that malaria was most common during the wintertime. Both groups also relied on a number of traditional remedies to alleviate symptoms, which were used as primary treatment, with formal healthcare treatment typically only sought when the illness progressed. ConclusionsThese results highlight significant differences between local cultural models and biomedical ones that could be detrimental to malaria eradication efforts. Kunene pastoralists have limited understanding of the causes of malaria, and beliefs about environmental and dietary causes may undermine attempts at prevention. Seeking healthcare solutions to malaria was normative, but secondary to use of at home traditional remedies. These findings indicate public health outreach and information campaigns are needed, particularly in rural groups with less formal education.more » « less

Summary Malaria is an infectious disease affecting a large population across the world, and interventions need to be efficiently applied to reduce the burden of malaria. We develop a framework to help policy-makers decide how to allocate limited resources in realtime for malaria control. We formalize a policy for the resource allocation as a sequence of decisions, one per intervention decision, that map up-to-date disease related information to a resource allocation. An optimal policy must control the spread of the disease while being interpretable and viewed as equitable to stakeholders. We construct an interpretable class of resource allocation policies that can accommodate allocation of resources residing in a continuous domain and combine a hierarchical Bayesian spatiotemporal model for disease transmission with a policy-search algorithm to estimate an optimal policy for resource allocation within the pre-specified class. The estimated optimal policy under the proposed framework improves the cumulative long-term outcome compared with naive approaches in both simulation experiments and application to malaria interventions in the Democratic Republic of the Congo.more » « less

NA (Ed.)Abstract In 1843, a hitherto unknown plant pathogen entered the US and spread to potato fields in the northeast. By 1845, the pathogen had reached Ireland leading to devastating famine. Questions arose immediately about the source of the outbreaks and how the disease should be managed. The pathogen, now known asPhytophthora infestans, still continues to threaten food security globally. A wealth of untapped knowledge exists in both archival and modern documents, but is not readily available because the details are hidden in descriptive text. In this work, we (1) used text analytics of unstructured historical reports (1843–1845) to map US late blight outbreaks; (2) characterized theories on the source of the pathogen and remedies for control; and (3) created modern late blight intensity maps using Twitter feeds. The disease spread from 5 to 17 states and provinces in the US and Canada between 1843 and 1845. Crop losses, Andean sources of the pathogen, possible causes and potential treatments were discussed. Modern disease discussion on Twitter included near-global coverage and local disease observations. Topic modeling revealed general disease information, published research, and outbreak locations. The tools described will help researchers explore and map unstructured text to track and visualize pandemics.more » « less

ABSTRACT ObjectiveInflammation may be an integral physiological mechanism through which discrimination impacts cardiovascular health and contributes to racial health disparities. Limited research has examined psychosocial factors that protect against the negative effects of discrimination on inflammation. Perceived control is a promising possible protective factor, given that it has been shown to moderate the relationship between other psychosocial stressors and physiological outcomes. This study thus tested whether systemic inflammation mediated the link between discrimination and cardiovascular health and whether perceived control moderated this relationship. MethodsData for this project included 347 non-Hispanic/Latinx Black adults (mean [standard deviation] age = 51.64 [11.24] years; 33% female) taken from the Midlife in the United States study. Perceived control and daily discrimination were assessed via self-report, and inflammation was measured via circulating levels of C-reactive protein (CRP), interleukin-6 (IL-6), fibrinogen, and tumor necrosis factor α. Cardiovascular health was measured by morbidity of cardiovascular conditions: heart disease, hypertension, and/or stroke. ResultsCRP (indirect effect:b =0.004, 95% confidence interval [CI] = 0.001–0.007) and fibrinogen (indirect effect:b =0.002, 95% CI = 0.0003–0.005) mediated the link between discrimination and cardiovascular conditions. Perceived control moderated the relationship between discrimination and CRP (F(1, 293) = 4.58, ΔR2= 0.013,b= −0.02, SE = 0.01,p= .033). CRP mediated the link between discrimination and cardiovascular conditions only for those who reported low levels of perceived control (Index = −0.003, 95% CI = −0.007 to −0.0001). ConclusionFindings provide empirical evidence of inflammation as a mechanism linking discrimination to cardiovascular conditions among Black Americans. Additionally, perceived control may be protective. Findings could suggest beliefs about control as a potential intervention target to help reduce the negative effects of discrimination on cardiovascular health among Black Americans.more » « less
