Title: Analyzing and predicting short-term substance use behaviors of persons who use drugs in the great plains of the U.S.
BackgroundSubstance use induces large economic and societal costs in the U.S. Understanding the change in substance use behaviors of persons who use drugs (PWUDs) over time, therefore, is important in order to inform healthcare providers, policymakers, and other stakeholders toward more efficient allocation of limited resources to at-risk PWUDs. ObjectiveThis study examines the short-term (within a year) behavioral changes in substance use of PWUDs at the population and individual levels. Methods237 PWUDs in the Great Plains of the U.S. were recruited by our team. The sample provides us longitudinal survey data regarding their individual attributes, including drug use behaviors, at two separate time periods spanning 4-12 months. At the population level, we analyze our data quantitatively for 18 illicit drugs; then, at the individual level, we build interpretable machine learning logistic regression and decision tree models for identifying relevant attributes to predict, for a given PWUD, (i) which drug(s) they would likely use and (ii) which drug(s) they would likely increase usage within the next 12 months. All predictive models were evaluated by computing the (averaged) Area under the Receiver Operating Characteristic curve (AUROC) and Area under the Precision-Recall curve (AUPR) on multiple distinct sets of hold-out sample. ResultsAt the population level, the extent of usage change and the number of drugs exhibiting usage changes follow power-law distributions. At the individual level, AUROC’s of the models for the top-4 prevalent drugs (marijuana, methamphetamines, amphetamines, and cocaine) range 0.756-0.829 (+2.88-7.66% improvement with respect to baseline models using only current usage of the respective drugs as input) for (i) and 0.670-0.765 (+4.34-18.0%) for (ii). The corresponding AUPR’s of the said models range 0.729-0.947 (+2.49-13.6%) for (i) and 0.348-0.618 (+26.9-87.6%) for (ii). ConclusionThe observed qualitative changes in short-term substance usage and the trained predictive models for (i) and (ii) can potentially inform human decision-making toward efficient allocation of appropriate resources to PWUDs at highest risk.  more » « less

  1. Substance use is a global issue that negatively impacts millions of persons who use drugs (PWUDs). In practice, identifying vulnerable PWUDs for efficient allocation of appropriate resources is challenging due to their complex use patterns (e.g., their tendency to change usage within months) and the high acquisition costs for collecting PWUD-focused substance use data. Thus, there has been a paucity of machine learning models for accurately predicting short-term substance use behaviors of PWUDs. In this paper, using longitudinal survey data of 258 PWUDs in the U.S. Great Plains collected by our team, we design a novel GAN that deals with high-dimensional low-sample-size tabular data and survey skip logic to augment existing data to improve classification models' prediction on (A) whether the PWUDs would increase usage and (B) at which ordinal frequency they would use a particular drug within the next 12 months. Our evaluation results show that, when trained on augmented data from our proposed GAN, the classification models improve their predictive performance (AUROC) by up to 13.4% in Problem (A) and 15.8% in Problem (B) for usage of marijuana, meth, amphetamines, and cocaine, which outperform state-of-the-art generative models.

  2. ImportanceEfforts to understand the complex association between social media use and mental health have focused on depression, with little investigation of other forms of negative affect, such as irritability and anxiety. ObjectiveTo characterize the association between self-reported use of individual social media platforms and irritability among US adults. Design, Setting, and ParticipantsThis survey study analyzed data from 2 waves of the COVID States Project, a nonprobability web-based survey conducted between November 2, 2023, and January 8, 2024, and applied multiple linear regression models to estimate associations with irritability. Survey respondents were aged 18 years and older. ExposureSelf-reported social media use. Main Outcomes and MeasuresThe primary outcome was score on the Brief Irritability Test (range, 5-30), with higher scores indicating greater irritability. ResultsAcross the 2 survey waves, there were 42 597 unique participants, with mean (SD) age 46.0 (17.0) years; 24 919 (58.5%) identified as women, 17 222 (40.4%) as men, and 456 (1.1%) as nonbinary. In the full sample, 1216 (2.9%) identified as Asian American, 5939 (13.9%) as Black, 5322 (12.5%) as Hispanic, 624 (1.5%) as Native American, 515 (1.2%) as Pacific Islander, 28 354 (66.6%) as White, and 627 (1.5%) as other (ie, selecting the other option prompted the opportunity to provide a free-text self-description). In total, 33 325 (78.2%) of the survey respondents reported daily use of at least 1 social media platform, including 6037 (14.2%) using once a day, 16 678 (39.2%) using multiple times a day, and 10 610 (24.9%) using most of the day. Frequent use of social media was associated with significantly greater irritability in univariate regression models (for more than once a day vs never, 1.43 points [95% CI, 1.22-1.63 points]; for most of the day vs never, 3.37 points [95% CI, 3.15-3.60 points]) and adjusted models (for more than once a day, 0.38 points [95% CI, 0.18-0.58 points]; for most of the day, 1.55 points [95% CI, 1.32-1.78 points]). These associations persisted after incorporating measures of political engagement. Conclusions and RelevanceIn this survey study of 42 597 US adults, irritability represented another correlate of social media use that merits further characterization, in light of known associations with depression and suicidality.

  3. Background:Ketamine has emerged as one of the most promising therapies for treatment-resistant depression. However, inter-individual variability in response to ketamine is still not well understood and it is unclear how ketamine’s molecular mechanisms connect to its neural and behavioral effects. Methods:We conducted a single-blind placebo-controlled study, with participants blinded to their treatment condition. 40 healthy participants received acute ketamine (initial bolus 0.23 mg/kg, continuous infusion 0.58 mg/kg/hr). We quantified resting-state functional connectivity via data-driven global brain connectivity and related it to individual ketamine-induced symptom variation and cortical gene expression targets. Results:We found that: (i) both the neural and behavioral effects of acute ketamine are multi-dimensional, reflecting robust inter-individual variability; (ii) ketamine’s data-driven principal neural gradient effect matched somatostatin (SST) and parvalbumin (PVALB) cortical gene expression patterns in humans, while the mean effect did not; and (iii) behavioral data-driven individual symptom variation mapped onto distinct neural gradients of ketamine, which were resolvable at the single-subject level. Conclusions:These results highlight the importance of considering individual behavioral and neural variation in response to ketamine. They also have implications for the development of individually precise pharmacological biomarkers for treatment selection in psychiatry.   4. Abstract Objective:Comprehensive studies examining longitudinal predictors of dietary change during the coronavirus disease 2019 pandemic are lacking. Based on an ecological framework, this study used longitudinal data to test if individual, social and environmental factors predicted change in dietary intake during the peak of the coronavirus 2019 pandemic in Los Angeles County and examined interactions among the multilevel predictors. Design:We analysed two survey waves (e.g. baseline and follow-up) of the Understanding America Study, administered online to the same participants 3 months apart. The surveys assessed dietary intake and individual, social, and neighbourhood factors potentially associated with diet. Lagged multilevel regression models were used to predict change from baseline to follow-up in daily servings of fruits, vegetables and sugar-sweetened beverages. Setting:Data were collected in October 2020 and January 2021, during the peak of the coronavirus disease 2019 pandemic in Los Angeles County. Participants:903 adults representative of Los Angeles County households. Results:Individuals who had depression and less education or who identified as non-Hispanic Black or Hispanic reported unhealthy dietary changes over the study period. Individuals with smaller social networks, especially low-income individuals with smaller networks, also reported unhealthy dietary changes. After accounting for individual and social factors, neighbourhood factors were generally not associated with dietary change. Conclusions:Given poor diets are a leading cause of death in the USA, addressing ecological risk factors that put some segments of the community at risk for unhealthy dietary changes during a crisis should be a priority for health interventions and policy.

  5. Abstract BackgroundAdolescents in foster care report high rates of mental health needs, yet intervention access remains limited. Substance use commonly co-occurs with mental health symptoms, but availability of substance use services for foster youth is even more scant than mental health services. Technology has advanced access to behavioral health care across the lifespan, but only for certain sectors of the population. Little research focuses on leveraging technology to advance access for foster youth. We report open trial findings, as a precursor to launching a large-scale implementation science trial, on how a U.S. nationwide serving support system for foster youth, Court Appointed Special Advocates (CASA), might be leveraged to expand access to substance use prevention resources via the FostrSpace app. FostrSpace provides asynchronous resources and synchronous navigator, peer support, and direct clinical intervention. A concurrent 6-session ECHO® substance use prevention telementoring curriculum was co-developed as a FostrSpace implementation strategy with a 6-member CASA Advisory Board. MethodsSeven youth-CASA dyads enrolled in the open trial. We used a mixed-methods design (quantitative assessment and qualitative exit interviews) to assess feasibility and acceptability of ECHO® sessions (CASA-only) and the usability of the FostrSpace app (youth-only). ResultsSix of seven of the youth accessed the app at least once, but a majority reported the app log-in process was burdensome and unappealing, thereby limiting them from frequently using the app. All youth rated the app features, design and content as appealing, helpful and relevant. ECHO® -FostrSpace session attendance was high (most attended 5 or more sessions) and CASAs found the content highly engaging and useful, especially regarding CASA-youth substance use communication skills. ConclusionsTechnological barriers, such as log-in burden, can prevent youth in need from accessing relevant services and must be regularly assessed and resolved. Substance use education and skills-building for CASAs is novel and a viable implementation strategy to increase foster youth access to digital behavioral health services innovations. Substance use prevention content should be integrated within discussions on youth mental health and trauma to be most engaging and relevant. Findings are informing the subsequent hybrid implementation-effectiveness trial design of FostrSpace with 400 youth-CASA dyads across 10 CASA programs in California.
