Title: Sleep program efficacy across gender and racial/ethnic groups: A content review and meta-analysis
Female and under-represented minority students (URMs) in higher education disproportionately experience sleep disturbances. Incorporating sleep education and other psychological interventions (e.g., cognitive behavioral therapy for insomnia; CBT-I) into college settings might provide a scalable solution, but it is unknown whether such psychoeducational interventions produce equivalent outcomes across groups. We addressed this question with two approaches. First, we identified 36 US-based studies that had student participants and a psychoeducational sleep intervention. Fifteen authors shared data on sleep knowledge, quality, and/or duration separated by gender and URM groups (N = 599). Hedges’ g was computed for each group within each study. Second, we systematically reviewed the content of 21 available programs. We found that, relative to national collegiate data (59.4 % female, 41.5 % URM), females were over-represented (63.4 %) and URM groups were under-represented (23.7 %) in these studies. Interventions improved sleep knowledge (g = 0.69, p<.001) and sleep quality (g = 0.25, p<.001), but not sleep duration (g = 0.13, p = .18). Effect sizes were similar in males, females, URM students, and non-URM students. There were no studies that incorporated financial, social, or neighborhood-level barriers to sleep in students. Future interventions in student groups might be improved by combining individualized, CBT-I-based education while concomitantly addressing environmental/institutional barriers to sleep.  more » « less

  1. Abstract Study Objective We investigated sleep disparities and academic achievement in college. Methods Participants were 6,002 first-year college students attending a midsize private university in the southern United States [62.0% female, 18.8% first-generation, 37.4% Black, Indigenous, or People of Color (BIPOC) students]. During the first 3–5 weeks of college, students reported their typical weekday sleep duration, which we classified as short sleep (<7 hours), normal sleep (7–9 hours), or long sleep (>9 hours). Results The odds for short sleep were significantly greater in BIPOC students (95% CI: 1.34–1.66) and female students (95% CI: 1.09–1.35), and the odds for long sleep were greater in BIPOC students (95% CI: 1.38–3.08) and first-generation students (95% CI: 1.04–2.53). In adjusted models, financial burden, employment, stress, STEM academic major, student athlete status, and younger age explained unique variance in sleep duration, fully mediating disparities for females and first-generation students (but only partially mediating disparities for BIPOC students). Short and long sleep predicted worse GPA across students’ first year in college, even after controlling for high school academic index, demographics, and psychosocial variables. Conclusions Higher education should address sleep health early in college to help remove barriers to success and reduce disparities.

  2. Background Shift workers are at high risk of developing sleep disorders such as shift worker sleep disorder or chronic insomnia. Cognitive behavioral therapy (CBT) is the first-line treatment for insomnia, and emerging evidence shows that internet-based CBT is highly effective with additional features such as continuous tracking and personalization. However, there are limited studies on internet-based CBT for shift workers with sleep disorders. Objective This study aimed to evaluate the impact of a 4-week, physician-assisted, internet-delivered CBT program incorporating machine learning–based well-being prediction on the sleep duration of shift workers at high risk of sleep disorders. We evaluated these outcomes using an internet-delivered CBT app and fitness trackers in the intensive care unit. Methods A convenience sample of 61 shift workers (mean age 32.9, SD 8.3 years) from the intensive care unit or emergency department participated in the study. Eligible participants were on a 3-shift schedule and had a Pittsburgh Sleep Quality Index score ≥5. The study comprised a 1-week baseline period, followed by a 4-week intervention period. Before the study, the participants completed questionnaires regarding the subjective evaluation of sleep, burnout syndrome, and mental health. Participants were asked to wear a commercial fitness tracker to track their daily activities, heart rate, and sleep for 5 weeks. The internet-delivered CBT program included well-being prediction, activity and sleep chart, and sleep advice. A job-based multitask and multilabel convolutional neural network–based model was used for well-being prediction. Participant-specific sleep advice was provided by sleep physicians based on daily surveys and fitness tracker data. The primary end point of this study was sleep duration. For continuous measurements (sleep duration, steps, etc), the mean baseline and week-4 intervention data were compared. The 2-tailed paired t test or Wilcoxon signed rank test was performed depending on the distribution of the data. Results In the fourth week of intervention, the mean daily sleep duration for 7 days (6.06, SD 1.30 hours) showed a statistically significant increase compared with the baseline (5.54, SD 1.36 hours; P=.02). Subjective sleep quality, as measured by the Pittsburgh Sleep Quality Index, also showed statistically significant improvement from baseline (9.10) to after the intervention (7.84; P=.001). However, no significant improvement was found in the subjective well-being scores (all P>.05). Feature importance analysis for all 45 variables in the prediction model showed that sleep duration had the highest importance. Conclusions The physician-assisted internet-delivered CBT program targeting shift workers with a high risk of sleep disorders showed a statistically significant increase in sleep duration as measured by wearable sensors along with subjective sleep quality. This study shows that sleep improvement programs using an app and wearable sensors are feasible and may play an important role in preventing shift work–related sleep disorders.   3. Study Objectives: Sleep education programs are emerging in university settings, but educating students about sleep health does not always change nighttime behaviors. A potential solution for bridging this knowledge-behavior gap is to incorporate behavioral change techniques into sleep education programs. This study’s objective was to test the implementation intention technique in students completing an academically-demanding gateway course for medical career paths. Methods: Participants were undergraduate students enrolled in organic chemistry courses (n = 101, 64.4% females, 51.5% nonwhite). After completing baseline questionnaires and actigraphy monitoring, participants took the Sleep 101 educational program. They were randomly assigned to an Education-Only condition or to also form an implementation intention (combining sleep education with an implementation intention strategy [Education+II]). Education+II individuals formed specific plans to go to bed earlier for the next 5 school nights. Participants repeated questionnaires at midsemester (mean = 50.33 days) and next-semester (mean = 248.51 days) follow-ups. Academic performance records were extracted prior to the study, during the study semester, and during the following semester. Results: Following the educational program, both groups reported improvements in sleep quality, but only Education+II participants showed actigraphymeasured improvements in bedtimes and sleep duration. Some effects in the Education+II condition persisted at midsemester and next-semester follow-ups. Adjusting for prior academic performance, the Education+II condition showed worse academic outcomes initially (but not long term) than the Education-Only condition, particularly among students with evening circadian preferences. Conclusions: Incorporating behavioral change strategies into sleep education programs can improve sleep outcomes. The relationship between sleep and academic performance is complex in demanding academic programs.

  4. We investigated the factors that predispose or precipitate greater intra-individual variability (IIV) in sleep. We further examined the potential consequences of IIV to overall sleep quality and health outcomes, including whether these relationships were found in both self-reported and actigraphy-measured sleep IIV. In Study 1, 699 US adults completed a Sleep Intra-Individual Variability questionnaire and self-reported psychosocial, sleep quality, and health outcomes. In Study 2, 100 university students wore actigraphy and completed psychosocial, sleep, and health surveys at multiple timepoints. In cross-sectional analyses that controlled for mean sleep duration, predisposing/precipitating factors to greater IIV were being an under-represented racial/ethnic minority, having greater stress or trait vulnerability to stress, and showing poorer time management. In addition, both studies showed that greater sleep IIV was associated with decreased overall sleep quality, independent of mean sleep duration. Concordance across subjective and objective IIV measures was modest and similar to concordance observed for subjective-objective mean sleep duration measures. Risk for irregular sleep patterns is increased in specific demographic groups and may be precipitated by, or contribute to, higher stress and time management inefficiencies. Irregular sleep may lead to poor sleep quality and adverse health outcomes, independent of mean sleep duration, underscoring the importance of addressing sleep consistency.

  5. Diversity in ocean sciences lags far behind US demographics. A substantial body of research highlights barriers that limit the participation and success of students from underrepresented minoritized (URM) and other marginalized groups in STEM disciplines, and a wealth of studies highlight successful interventions that improve the persistence of these groups in STEM higher education. Despite this knowledge, over the past four decades, ocean sciences has made limited progress in growing diversity within its workforce, suggesting new strategies are needed. Undergraduate research experiences are a pivotal pathway toward graduate education and future careers in ocean sciences, but they are plagued by many issues that limit the participation and success of persons from URM and other marginalized backgrounds. Here we summarize obstacles that limit participation of diverse populations in ocean sciences and highlight successful strategies for overcoming these obstacles. By re-envisioning how we approach undergraduate research experiences and bringing intentionality to the recruitment of students and the training environments that they experience, we can more effectively grow diversity in ocean sciences and unleash the power of diversity to address the pressing local and global problems facing marine ecosystems.
