Title: Changes over Time in Association Patterns between Estimated COVID-19 Case Fatality Rates and Demographic, Socioeconomic and Health Factors in the US States of Florida and New York
The United States struggled exceptionally during the COVID-19 pandemic. For researchers and policymakers, it is of great interest to understand the risk factors associated with COVID-19 when examining data aggregated at a regional level. We examined the county-level association between the reported COVID-19 case fatality rate (CFR) and various demographic, socioeconomic and health factors in two hard-hit US states: New York and Florida. In particular, we examined the changes over time in the association patterns. For each state, we divided the data into three seasonal phases based on observed waves of the COVID-19 outbreak. For each phase, we used tests of correlations to explore the marginal association between each potential covariate and the reported CFR. We used graphical models to further clarify direct or indirect associations in a multivariate setting. We found that during the early phase of the pandemic, the association patterns were complex: the reported CFRs were high, with great variation among counties. As pandemics progressed, especially during the winter phase, socioeconomic factors such as median household income and health-related factors such as the prevalence of adult smokers and mortality rate of respiratory diseases became more significantly associated with the CFR.   1. Limited research has been conducted on the mental health concerns of frontline and essential workers and their children during the COVID-19 pandemic in the United States (U.S.). This study examined the association between working on the frontlines in the U.S. during the COVID-19 pandemic (March to July 2020) and personal crisis text concerns (e.g., self-harm, suicidal thoughts, anxiety/stress, and substance abuse) for frontline essential workers and the children of frontline workers. We used a novel data set from a crisis texting service, Crisis Text Line (CTL), that is widely used throughout the U.S. Generalized Estimating Equations examined the individual association between eight specific crisis types (Depression, Stress/Anxiety, Self-Harm, Suicidal Thoughts, Substance Abuse, Isolation, Relationship Issues, and Abuse) and being in frontline work or being a child of a frontline worker during the early phase of the pandemic. Using CTL concerns as a proxy for the prevalence of mental health issues, we found that children of workers, specifically the youngest demographic (13 years and under), females, and non-conforming youth had a higher risk of specific crisis events during the COVID-19 pandemic. Additionally, Hispanic children of workers reported higher rates of stress/anxiety, whereas African American children of workers had higher rates of abuse and depression. Frontline workers had a higher risk of suicidal thoughts, and the risk of crisis events was generally highest for non-binary, transgender, and male users. Increases in CTL usage among frontline workers were noted across 7–28 days after spikes in local COVID-19 cases. The research to date has focused on the mental health of frontline essential workers, but our study highlights troubling trends in psychological stress among children of these workers. Supportive interventions and mental health resources are needed not only for frontline essential workers but for their children too.

  2. Background: The health belief model suggests that individuals' beliefs affect behaviors associated with health. This study examined whether Ohioans' pre-existing medical health diagnoses affected their belief about personal health risk and their compliance with social distancing during the coronavirus disease 2019 (COVID-19) pandemic. Prior research examining physical and mental diagnoses and social distancing compliance is nearly nonexistent. We examined whether physical and mental health diagnoses influenced individuals' beliefs that their health is at risk and their adherence with social distancing guidelines. Methods: The study used longitudinal cohort data from the Toledo Adolescent Relationships Study (TARS) (n = 790), which surveyed Ohioans prior to and during the COVID-19 pandemic. Dependent variables included belief that an individual's own health was at risk and social distancing compliance. Independent variables included physical and mental health diagnoses, pandemic-related factors (fear of COVID-19, political beliefs about the pandemic, friends social distance, family social distance, COVID-19 exposure), and sociodemographic variables (age, gender, race/ethnicity, educational level). Results: Individuals who had a pre-existing physical health diagnosis were more likely to believe that their personal health was at risk during the pandemic but were not more likely to comply with social distancing guidelines. In contrast, individuals who had a pre-existing mental health diagnosis were more compliant with social distancing guidelines but were not more likely to believe their personal health was at risk. Individuals who expressed greater fear of COVID-19 believed their health is more at risk than those who expressed lower levels of fear. Conclusion: Health considerations are important to account for in assessments of responses to the pandemic, beliefs about personal health risk, and social distancing behavior. Additional research is needed to understand the divergence in the findings regarding physical health, beliefs about personal health risk, and social distancing compliance. Further, research is needed to understand how mental health issues impact decision-making related to social distancing compliance.

  3. Bristol Bay in Alaska is home to the world’s largest commercial salmon fishery. During an average fishing season, the population of the Bristol Bay region more than doubles as thousands of workers from out of state converge on the fishery. In the months leading up to the 2020 commercial fishery opening, as the COVID-19 pandemic exploded worldwide, great uncertainty existed about the health risks of opening the fishery. Bristol Bay residents had not yet experienced any cases of COVID-19, yet the livelihoods of most were closely tied to the commercial fishery opening. To better understand how COVID-19 risk perceptions affected decisions to participate in the fishery, we administered an online survey to community members and fishery participants. We collected standard socioeconomic data and posed questions to gauge risk perceptions related to COVID-19. We find that COVID-19 risk perceptions vary across race/ethnic groups by residency and income. People with below median income who are members of minority groups—notably, non-resident Hispanic workers and resident Alaska Native respondents—reported the highest risk perceptions related to COVID-19. This study highlights the important linkages among risk perceptions, socioeconomic characteristics, and employment decisions during an infectious disease outbreak.

  4. Abstract The COVID-19 pandemic brought unprecedented changes to various aspects of daily life, profoundly affecting human mobility. These changes in mobility patterns were not uniform, as numerous factors, including public health measures, socioeconomic status, and urban infrastructure, influenced them. This study examines human mobility changes during COVID-19 in San Diego County and New York City, employing Latent Profile Analysis (LPA) and various network measures to analyze connectivity and socioeconomic status (SES) within these regions. While many COVID-19 and mobility studies have revealed overall reductions in mobility or changes in mobility patterns, they often fail to specify ’where’ these changes occur and lack a detailed understanding of the relationship between SES and mobility changes. This creates a significant research gap in understanding the spatial and socioeconomic dimensions of mobility changes during the pandemic. This study aims to address this gap by providing a comprehensive analysis of how mobility patterns varied across different socioeconomic groups during the pandemic. By comparing mobility patterns before and during the pandemic, we aim to shed light on how this unprecedented event impacted different communities. Our research contributes to the literature by employing network science to examine COVID-19’s impact on human mobility, integrating SES variables into the analysis of mobility networks. This approach provides a detailed understanding of how social and economic factors influence movement patterns and urban connectivity, highlighting disparities in mobility and access across different socioeconomic groups. The results identify areas functioning as hubs or bridges and illustrate how these roles changed during COVID-19, revealing existing societal inequalities. Specifically, we observed that urban parks and rural areas with national parks became significant mobility hubs during the pandemic, while affluent areas with high educational attainment saw a decline in centrality measures, indicating a shift in urban mobility dynamics and exacerbating pre-existing socioeconomic disparities.

  5. Purpose: The first year of the COVID-19 pandemic constituted a major life stress event for many adolescents, associated with disrupted school, behaviors, social networks, and health concerns. However, pandemic-related stress was not equivalent for everyone and could have been influenced by pre-pandemic factors including brain structure and sleep, which both undergo substantial development during adolescence. Here, we analyzed clusters of perceived stress levels across the pandemic and determined developmentally relevant pre-pandemic risk factors in brain structure and sleep of persistently high stress during the first year of the COVID-19 pandemic. Methods: We investigated longitudinal changes in perceived stress at six timepoints across the first year of the pandemic (May 2020–March 2021) in 5559 adolescents (50 % female; age range: 11–14 years) in the United States (U.S.) participating in the Adolescent Brain Cognitive Development (ABCD) study. In 3141 of these adolescents, we fitted machine learning models to identify the most important pre-pandemic predictors from structural MRI brain measures and self-reported sleep data that were associated with persistently high stress across the first year of the pandemic. Results: Patterns of perceived stress levels varied across the pandemic, with 5 % reporting persistently high stress. Our classifiers accurately detected persistently high stress (AUC > 0.7). Pre-pandemic brain structure, specifically cortical volume in temporal regions, and cortical thickness in multiple parietal and occipital regions, predicted persistent stress. Pre-pandemic sleep difficulties and short sleep duration were also strong predictors of persistent stress, along with more advanced pubertal stage. Conclusions: Adolescents showed variable stress responses during the first year of the COVID-19 pandemic, and some reported persistently high stress across the whole first year. Vulnerability to persistent stress was evident in several brain structural and self-reported sleep measures, collected before the pandemic, suggesting the relevance of other pre-existing individual factors beyond pandemic-related factors, for persistently high stress responses.
