Title: How the COVID-19 pandemic reshaped demographic variation in mental health among diverse engineering student populations
Mental health issues have long posed a challenge on university campuses. While no population is immune, research has shown that students from marginalised backgrounds can have higher rates of mental health issues and suffer worse outcomes as a result. These discrepancies have been attributed to everything from different cultural norms to the micro-aggressions and other barriers that students from marginalised populations face on university campuses. With the onset of COVID-19 in the United States, many residential universities switched to a remote learning model, fundamentally changing the relationship between students, campus, family support. This work uses survey data from students in the United States to explore how COVID-19 affected mental health issues among students from different backgrounds. While the pandemic drastically increased rates of depressive disorder among all respondents, discrepancies between mental health rates for women and Hispanic/Latinx compared to men and White respondents either decreased or disappeared. Additionally, respondents identifying as Asians were less likely to screen positive for several mental health conditions than White, Non-Hispanic respondents. These findings may point to important new insights about the ways in which engineering education undermines some groups’ mental health.  more » « less

    ABSTRACT The global spread of the novel coronavirus first reported in December 2019 led to drastic changes in the social and economic dynamics of everyday life. Nationwide, racial, gender, and geographic disparities in symptom severity, mortality, and access to health care evolved, which impacted stress and anxiety surrounding COVID-19. On university campuses, drastic shifts in learning environments occurred as universities shifted to remote instruction, which further impacted student mental health and anxiety. Our study aimed to understand how students from diverse backgrounds differ in their worry and stress surrounding COVID-19 upon return to hybrid or in-person classes during the Fall of 2020. Specifically, we addressed the differences in COVID-19 worry, stress response, and COVID-19-related food insecurity related to race/ethnicity (Indigenous American, Asian/Asian American, black/African American, Latinx/Hispanic, white, or multiple races), gender (male, female, and gender expressive), and geographic origin (ranging from rural to large metropolitan areas) of undergraduate students attending a regional-serving R2 university, in the southeastern U.S. Overall, we found significance in worry, food insecurity, and stress responses with females and gender expressive individuals, along with Hispanic/Latinx, Asian/Asian American, and black/African American students. Additionally, students from large urban areas were more worried about contracting the virus compared to students from rural locations. However, we found fewer differences in self-reported COVID-related stress responses within these students. Our findings can highlight the disparities among students’ worry based on gender, racial differences, and geographic origins, with potential implications for mental health of university students from diverse backgrounds. Our results support the inclusion of diverse voices in university decisioning making around the transition through the COVID-19 pandemic.

  2. The early phases of the COVID-19 pandemic in the United States caused unprecedented disruption to engineering students and society-at-large. Residential students suddenly found themselves forced off-campus and into a new regime of online learning. On top of this, students faced pandemic related uncertainty about their health and the health of their loved ones, restrictions on social life, and the privations of a stuttering economy. It is perhaps unsurprising, then, that during the early days of the COVID-19 pandemic the engineering student population saw large increases in incidences of depression and other psychological conditions. While COVID-19 continues to be a concern, many of the restrictions and precautions associated with the early outbreak have been relaxed. Most engineering programs are back to in person learning, and strict COVID-testing regimes, mask mandates, and limits on large public gatherings have been largely phased out. With academia and society slowly adjusting to this “new normal,” it is important to know whether and to what extent mental health of engineering students has changed over the course of the pandemic. This work explores this question by analyzing longitudinal data collected at four times from 2019-2021. We analyze how the prevalence of different conditions changed with the onset of the COVID-19 pandemic, and whether they have returned to pre-pandemic levels. Among other results, we found statistically significant increases were observed in total depressive disorders between our initial sample (Fall 2019) and a sample taken immediately post-COVID (Spring 2020). While measured rates of depression decreased between Spring 2020 and Fall 2021, it was not statistically significant, potentially indicating that the student population is still recovering on this metric. Conversely our data shows a statistically significant drop in moderate-to-major psychological distress between our Fall 2019 pre-pandemic sampling and our Fall 2021 post-pandemic sampling indicating, potentially indicating an improvement in overall mental health. Breaking the data down by gender, no significant changes were observed across any measure during the four sample periods for women respondents. Men, however, showed a significant increase in depressive disorders from Fall 2019 to Spring 2020; and a significant decrease in depressive disorders from Spring 2020 to Fall 2021.

  3. Limited research has been conducted on the mental health concerns of frontline and essential workers and their children during the COVID-19 pandemic in the United States (U.S.). This study examined the association between working on the frontlines in the U.S. during the COVID-19 pandemic (March to July 2020) and personal crisis text concerns (e.g., self-harm, suicidal thoughts, anxiety/stress, and substance abuse) for frontline essential workers and the children of frontline workers. We used a novel data set from a crisis texting service, Crisis Text Line (CTL), that is widely used throughout the U.S. Generalized Estimating Equations examined the individual association between eight specific crisis types (Depression, Stress/Anxiety, Self-Harm, Suicidal Thoughts, Substance Abuse, Isolation, Relationship Issues, and Abuse) and being in frontline work or being a child of a frontline worker during the early phase of the pandemic. Using CTL concerns as a proxy for the prevalence of mental health issues, we found that children of workers, specifically the youngest demographic (13 years and under), females, and non-conforming youth had a higher risk of specific crisis events during the COVID-19 pandemic. Additionally, Hispanic children of workers reported higher rates of stress/anxiety, whereas African American children of workers had higher rates of abuse and depression. Frontline workers had a higher risk of suicidal thoughts, and the risk of crisis events was generally highest for non-binary, transgender, and male users. Increases in CTL usage among frontline workers were noted across 7–28 days after spikes in local COVID-19 cases. The research to date has focused on the mental health of frontline essential workers, but our study highlights troubling trends in psychological stress among children of these workers. Supportive interventions and mental health resources are needed not only for frontline essential workers but for their children too.

  4. Importance

    Marked elevation in levels of depressive symptoms compared with historical norms have been described during the COVID-19 pandemic, and understanding the extent to which these are associated with diminished in-person social interaction could inform public health planning for future pandemics or other disasters.

    Objective

    To describe the association between living in a US county with diminished mobility during the COVID-19 pandemic and self-reported depressive symptoms, while accounting for potential local and state-level confounding factors.

    Design, Setting, and Participants

    This survey study used 18 waves of a nonprobability internet survey conducted in the United States between May 2020 and April 2022. Participants included respondents who were 18 years and older and lived in 1 of the 50 US states or Washington DC.

    Main Outcome and Measure

    Depressive symptoms measured by the Patient Health Questionnaire-9 (PHQ-9); county-level community mobility estimates from mobile apps; COVID-19 policies at the US state level from the Oxford stringency index.

    Results

    The 192 271 survey respondents had a mean (SD) of age 43.1 (16.5) years, and 768 (0.4%) were American Indian or Alaska Native individuals, 11 448 (6.0%) were Asian individuals, 20 277 (10.5%) were Black individuals, 15 036 (7.8%) were Hispanic individuals, 1975 (1.0%) were Pacific Islander individuals, 138 702 (72.1%) were White individuals, and 4065 (2.1%) were individuals of another race. Additionally, 126 381 respondents (65.7%) identified as female and 65 890 (34.3%) as male. Mean (SD) depression severity by PHQ-9 was 7.2 (6.8). In a mixed-effects linear regression model, the mean county-level proportion of individuals not leaving home was associated with a greater level of depression symptoms (β, 2.58; 95% CI, 1.57-3.58) after adjustment for individual sociodemographic features. Results were similar after the inclusion in regression models of local COVID-19 activity, weather, and county-level economic features, and persisted after widespread availability of COVID-19 vaccination. They were attenuated by the inclusion of state-level pandemic restrictions. Two restrictions, mandatory mask-wearing in public (β, 0.23; 95% CI, 0.15-0.30) and policies cancelling public events (β, 0.37; 95% CI, 0.22-0.51), demonstrated modest independent associations with depressive symptom severity.

    Conclusions and Relevance

    In this study, depressive symptoms were greater in locales and times with diminished community mobility. Strategies to understand the potential public health consequences of pandemic responses are needed.

This research full paper presents screening rates for mental health issues and life-stress events in engineering-focused community college students during the initial phases of the COVID-19 pandemic in the US. Specifically, it attempts to answer the following research questions: 1) What is the overall rate of various mental health conditions among engineering-focused community college students, 2) What effects has the pandemic had on baseline stress levels engineering-focused community college, and 3) What effects has the pandemic had on quality of life, such as sleep habits and financial security of engineering- focused community college students? Data for this paper was collected via survey from May–July 2020 and includes responses from 84 students at 24 community colleges. The survey itself was a compilation of several widely- used instruments for measuring overall mental health and stress levels in a population. These instruments include the Kessler-6 for psychological distress, the PHQ for anxiety, depression, and eating disorders, the PC-PTSD for PTSD-like symptoms, and the SRRS for inventorying stressful life events. Among the major findings, 32% of respondents reported a major change in financial situation, 27% reported loss of employment, and 13% reported ceasing formal schooling because of the COVID-19 pandemic. Additionally, 32% of respondents reported that the COVID-19 pandemic worsened their housing security situation, 38% reported that COVID-19 has worsened their food security situation, and 36% report that COVID-19 has decreased their ability to access instruction, course materials, or course supplies. Finally, of respondents who completed at least one mental health screening instrument, 70% screened positive for at least one potentially diagnosable condition, while only 9% reported ever receiving a mental health diagnosis.
