Title: Reimagining From Abroad: Speech-Language Pathologists as Interprofessional Partners Supporting Justice-Involved Youth in America
Purpose:With application to the United States, this tutorial explores barriers in the American juvenile justice system for justice-involved youth (JIY) with cognitive-communication disorders (CCDs). It outlines models from abroad and reimagines the American juvenile justice system to include speech-language pathologists (SLPs) as interprofessional practice partners. Method:Interprofessional (i.e., criminal justice, speech-language pathology) literature from the United States and overseas is reviewed and summarized to explain the American juvenile justice system, outline areas of concern for youth with CCDs, and describe potential solutions. Results:The application of speech-language pathology services within the juvenile justice system is explained and visually depicted. This framework was informed by intervention models and approaches from international examples. Conclusions:There is an opportunity to embed speech-language pathology services from intake into court action and through disposition for JIY with cognitive-communication impairments. This includes interprofessional education and development, SLPs providing direct intervention, and multidisciplinary screening efforts. Speech-language pathologists as interprofessional advocates and practice partners can improve life chances and outcomes for youth with CCDs in the juvenile justice system.  more » « less

  1. Purpose:The presented work was invited following the American Speech-Language-Hearing Association SIG 19 Virtual Talk “Speech Science in Diverse Populations” that occurred on September 2, 2021. The purpose of this article was to introduce the historical and theoretical frameworks of gender and race from a decidedly North American (United States) perspective to an audience that may be less familiar with those topics as they relate to the practice of communication sciences and disorders. Race and gender are huge topics. Entire fields of study and lifetimes of work are dedicated to understanding these constructs. Therefore, it is hoped that this brief review of race and gender will prompt the reader to evaluate how the two constructs are used to categorize people and whether being a member of a marginalized or a minoritized group affects the person's access to or use of intervention services. A critical theoretical discussion of race and gender is beyond the scope of this text. In this limited space, this work presents an overview of current and historical discussions of gender and race and a challenge to the reader to accept that their perspective is indebted to a specific belief system. In the United States, that belief system often evaluates human differences into binary categories on a weighted continuum. Speech-language professionals often use that continuum to identify and measure difference into either acceptable variation or disorder. Conclusions:The profession of speech-language pathology was established during a time when variation from middle-class White American communication norms was frequently defined as undesirable and sometimes as disordered. The communities and individuals we encounter deserve to be accepted as they are. We must resolve to expect and accept wide variation in human communication without pathologizing its existence, to expand our thinking about disorder in speech and hearing science, and to accept culturally competent communicators as competent communicators.

  2. Speech is a fundamental aspect of human life, crucial not only for communication but also for cognitive, social, and academic development. Children with speech disorders (SD) face significant challenges that, if unaddressed, can result in lasting negative impacts. Traditionally, speech and language assessments (SLA) have been conducted by skilled speech-language pathologists (SLPs), but there is a growing need for efficient and scalable SLA methods powered by artificial intelligence. This position paper presents a survey of existing techniques suitable for automating SLA pipelines, with an emphasis on adapting automatic speech recognition (ASR) models for children’s speech, an overview of current SLAs and their automated counterparts to demonstrate the feasibility of AI-enhanced SLA pipelines, and a discussion of practical considerations, including accessibility and privacy concerns, associated with the deployment of AI-powered SLAs.

  3. The emergence of COVID-19 placed immediate pressure on the juvenile justice system to adapt to changes in case processing and decision-making practices. Juvenile probation agencies were tasked with quickly altering their policies and practice to abide by local public health measures. As probation supervision is the most common disposition in the juvenile justice system, there is both an empirical and practical need to understand the impact that COVID-19 has on a variety of issues surrounding the supervision and provision of services for juveniles. Using self-report survey data from juvenile probation directors across the United States, the current study examines (a) the biggest challenges faced by juvenile probation agencies during the pandemic, (b) the strategies implemented in response to these challenges, and (c) the most pressing issues currently facing the field of juvenile community corrections. Results have the potential to inform future agency decision-making when adjusting juvenile probation policy and practice.

  4. Abstract BackgroundDiagnostic pathology depends on complex, structured reasoning to interpret clinical, histologic, and molecular data. Replicating this cognitive process algorithmically remains a significant challenge. As large language models (LLMs) gain traction in medicine, it is critical to determine whether they have clinical utility by providing reasoning in highly specialized domains such as pathology. MethodsWe evaluated the performance of four reasoning LLMs (OpenAI o1, OpenAI o3-mini, Gemini 2.0 Flash Thinking Experimental, and DeepSeek-R1 671B) on 15 board-style open-ended pathology questions. Responses were independently reviewed by 11 pathologists using a structured framework that assessed language quality (accuracy, relevance, coherence, depth, and conciseness) and seven diagnostic reasoning strategies. Scores were normalized and aggregated for analysis. We also evaluated inter-observer agreement to assess scoring consistency. Model comparisons were conducted using one-way ANOVA and Tukey’s Honestly Significant Difference (HSD) test. ResultsGemini and DeepSeek significantly outperformed OpenAI o1 and OpenAI o3-mini in overall reasoning quality (p < 0.05), particularly in analytical depth and coherence. While all models achieved comparable accuracy, only Gemini and DeepSeek consistently applied expert-like reasoning strategies, including algorithmic, inductive, and Bayesian approaches. Performance varied by reasoning type: models performed best in algorithmic and deductive reasoning and poorest in heuristic and pattern recognition. Inter-observer agreement was highest for Gemini (p < 0.05), indicating greater consistency and interpretability. Models with more in-depth reasoning (Gemini and DeepSeek) were generally less concise. ConclusionAdvanced LLMs such as Gemini and DeepSeek can approximate aspects of expert-level diagnostic reasoning in pathology, particularly in algorithmic and structured approaches. However, limitations persist in contextual reasoning, heuristic decision-making, and consistency across questions. Addressing these gaps, along with trade-offs between depth and conciseness, will be essential for the safe and effective integration of AI tools into clinical pathology workflows.

  5. Speech-language pathologists are familiar with eligibility criteria for school-based special education services under IDEA, the Individuals with Disabilities Education Act. In order for children with speech and language disorders to be eligible for services, they need to fit one of the thirteen categories of disabilities. However, these 13 categories do not always align well with current evidence-based diagnoses of neurodiverse conditions. It is because of these challenges that we, as members of the National Artificial Intelligence Institute for Exceptional Education, are particularly grateful to the US Department of Education's Office of Special Education programs for issuing new guidance on the use of DLD to accurately describe the speech and language needs of individual children, no matter what eligibility category they fall into. We are also grateful to members and leaders of the American Speech-Language-Hearing Association for their strong advocacy to raise the community's awareness of this new guidance. Therefore, our Institute will be another strong advocate for children with DLD so that they can eventually benefit from our Institute's research. We believe that the recognition of DLD as a disability can greatly help these children and their families.
