            Purpose To investigate relationships between blood pressure and the thickness of single retinal layers in the macula. Methods Participants of the population-based Beijing Eye Study, free of retinal or optic nerve disease, underwent medical and ophthalmological examinations including optical coherence tomographic examination of the macula. Applying a multiple-surface segmentation solution, we automatically segmented the retina into its various layers. Results The study included 2237 participants (mean age 61.8±8.4 years, range 50–93 years). Mean thicknesses of the retinal nerve fibre layer (RNFL), ganglion cell layer (GCL), inner plexiform layer, inner nuclear layer (INL), outer plexiform layer, outer nuclear layer/external limiting membrane, ellipsoid zone, photoreceptor outer segments (POS) and retinal pigment epithelium–Bruch membrane were 31.1±2.3 µm, 39.7±3.5 µm, 38.4±3.3 µm, 34.8±2.0 µm, 28.1±3.0 µm, 79.2±7.3 µm, 22.9±0.6 µm, 19.2±3.3 µm and 20.7±1.4 µm, respectively. In multivariable analysis, higher systolic blood pressure (SBP) and diastolic blood pressure (DBP) were associated with thinner GCL and thicker INL, after adjusting for age, sex and axial length (all p<0.0056). Higher SBP was additionally associated with thinner POS and higher DBP with thinner RNFL. For an elevation of SBP/DBP by 10 mm Hg, the RNFL, GCL, INL and POS changed by 2.0, 3.0, 1.5 and 2.0 µm, respectively. Conclusions Thickness of RNFL, GCL and POS was inversely and INL thickness was positively associated with higher blood pressure, while the thickness of the other retinal layers was not significantly correlated with blood pressure. The findings may be helpful for refinement of the morphometric detection of retinal diseases.more » « less

            Chaudhuri, Kamalika and (Ed.)In this paper, we propose systematic and efficient gradient-based methods for both one-way and two-way partial AUC (pAUC) maximization that are applicable to deep learning. We propose new formulations of pAUC surrogate objectives by using the distributionally robust optimization (DRO) to define the loss for each individual positive data. We consider two formulations of DRO, one of which is based on conditional-value-at-risk (CVaR) that yields a non-smooth but exact estimator for pAUC, and another one is based on a KL divergence regularized DRO that yields an inexact but smooth (soft) estimator for pAUC. For both one-way and two-way pAUC maximization, we propose two algorithms and prove their convergence for optimizing their two formulations, respectively. Experiments demonstrate the effectiveness of the proposed algorithms for pAUC maximization for deep learning on various datasets.more » « less

            Segmentation of multiple surfaces in optical coherence tomography (OCT) images is a challenging problem, further complicated by the frequent presence of weak boundaries, varying layer thicknesses, and mutual influence between adjacent surfaces. The traditional graph-based optimal surface segmentation method has proven its effectiveness with its ability to capture various surface priors in a uniform graph model. However, its efficacy heavily relies on handcrafted features that are used to define the surface cost for the “goodness” of a surface. Recently, deep learning (DL) is emerging as a powerful tool for medical image segmentation thanks to its superior feature learning capability. Unfortunately, due to the scarcity of training data in medical imaging, it is nontrivial for DL networks toimplicitlylearn the global structure of the target surfaces, including surface interactions. This study proposes to parameterize the surface cost functions in the graph model and leverage DL to learn those parameters. The multiple optimal surfaces are then simultaneously detected by minimizing the total surface cost whileexplicitlyenforcing the mutual surface interaction constraints. The optimization problem is solved by the primal-dual interior-point method (IPM), which can be implemented by a layer of neural networks, enabling efficient end-to-end training of the whole network. Experiments on spectral-domain optical coherence tomography (SD-OCT) retinal layer segmentation demonstrated promising segmentation results with sub-pixel accuracy.more » « less

            Abstract Non-small-cell lung cancer (NSCLC) represents approximately 80–85% of lung cancer diagnoses and is the leading cause of cancer-related death worldwide. Recent studies indicate that image-based radiomics features from positron emission tomography/computed tomography (PET/CT) images have predictive power for NSCLC outcomes. To this end, easily calculated functional features such as the maximum and the mean of standard uptake value (SUV) and total lesion glycolysis (TLG) are most commonly used for NSCLC prognostication, but their prognostic value remains controversial. Meanwhile, convolutional neural networks (CNN) are rapidly emerging as a new method for cancer image analysis, with significantly enhanced predictive power compared to hand-crafted radiomics features. Here we show that CNNs trained to perform the tumor segmentation task, with no other information than physician contours, identify a rich set of survival-related image features with remarkable prognostic value. In a retrospective study on pre-treatment PET-CT images of 96 NSCLC patients before stereotactic-body radiotherapy (SBRT), we found that the CNN segmentation algorithm (U-Net) trained for tumor segmentation in PET and CT images, contained features having strong correlation with 2- and 5-year overall and disease-specific survivals. The U-Net algorithm has not seen any other clinical information (e.g. survival, age, smoking history, etc.) than the images and the corresponding tumor contours provided by physicians. In addition, we observed the same trend by validating the U-Net features against an extramural data set provided by Stanford Cancer Institute. Furthermore, through visualization of the U-Net, we also found convincing evidence that the regions of metastasis and recurrence appear to match with the regions where the U-Net features identified patterns that predicted higher likelihoods of death. We anticipate our findings will be a starting point for more sophisticated non-intrusive patient specific cancer prognosis determination. For example, the deep learned PET/CT features can not only predict survival but also visualize high-risk regions within or adjacent to the primary tumor and hence potentially impact therapeutic outcomes by optimal selection of therapeutic strategy or first-line therapy adjustment.more » « less

            Optimal surface segmentation is a state-of-the-art method used for segmentation of multiple globally optimal surfaces in volumetric datasets. The method is widely used in numerous medical image segmentation applications. However, nodes in the graph based optimal surface segmentation method typically encode uniformly distributed orthogonal voxels of the volume. Thus the segmentation cannot attain an accuracy greater than a single unit voxel, i.e. the distance between two adjoining nodes in graph space. Segmentation accuracy higher than a unit voxel is achievable by exploiting partial volume information in the voxels which shall result in non-equidistant spacing between adjoining graph nodes. This paper reports a generalized graph based multiple surface segmentation method with convex priors which can optimally segment the target surfaces in an irregularly sampled space. The proposed method allows non-equidistant spacing between the adjoining graph nodes to achieve subvoxel segmentation accuracy by utilizing the partial volume information in the voxels. The partial volume information in the voxels is exploited by computing a displacement field from the original volume data to identify the subvoxel-accurate centers within each voxel resulting in non-equidistant spacing between the adjoining graph nodes. The smoothness of each surface modeled as a convex constraint governs the connectivity and regularity of the surface. We employ an edge-based graph representation to incorporate the necessary constraints and the globally optimal solution is obtained by computing a minimum s-t cut. The proposed method was validated on 10 intravascular multi-frame ultrasound image datasets for subvoxel segmentation accuracy. In all cases, the approach yielded highly accurate results. Our approach can be readily extended to higher-dimensional segmentations.more » « less

            Shape priors have been widely utilized in medical image segmentation to improve segmentation accuracy and robustness. A major way to encode such a prior shape model is to use a mesh representation, which is prone to causing self-intersection or mesh folding. Those problems require complex and expensive algorithms to mitigate. In this paper, we propose a novel shape prior directly embedded in the voxel grid space, based on gradient vector flows of a pre-segmentation. The flexible and powerful prior shape representation is ready to be extended to simultaneously segmenting multiple interacting objects with minimum separation distance constraint. The segmentation problem of multiple interacting objects with shape priors is formulated as a Markov Random Field problem, which seeks to optimize the label assignment (objects or background) for each voxel while keeping the label consistency between the neighboring voxels. The optimization problem can be efficiently solved with a single minimum s-t cut in an appropriately constructed graph. The proposed algorithm has been validated on two multi-object segmentation applications: the brain tissue segmentation in MRI images and the bladder/prostate segmentation in CT images. Both sets of experiments showed superior or competitive performance of the proposed method to the compared state-of-the-art methods.more » « less

            null (Ed.)Positron emission tomography and computed tomography (PET-CT) dual-modality imaging provides critical diagnostic information in modern cancer diagnosis and therapy. Automated accurate tumor delineation is essentially important in computer-assisted tumor reading and interpretation based on PET-CT. In this paper, we propose a novel approach for the segmentation of lung tumors that combines the powerful fully convolutional networks (FCN) based semantic segmentation framework (3D-UNet) and the graph cut based co-segmentation model. First, two separate deep UNets are trained on PET and CT, separately, to learn high level discriminative features to generate tumor/non-tumor masks and probability maps for PET and CT images. Then, the two probability maps on PET and CT are further simultaneously employed in a graph cut based co-segmentation model to produce the final tumor segmentation results. Comparative experiments on 32 PET-CT scans of lung cancer patients demonstrate the effectiveness of our method.more » « less
