Patient-generated data, such as recorded Observations of Daily Living (ODL) and Patient-Reported Outcomes (PRO) data, are valued sources of information in oncology care. However, prior work largely focuses on capturing clinician-defined, patient-generated data in adult oncology care. Emerging research at the intersection of human–computer interaction and medical informatics suggests that visual narratives of patients’ observations of daily living (Visual ODLs) could better support multi-party review of patients’ everyday symptoms and quality of life, potentially improving patient–clinician communication. In this paper, we build on a prior study of Visual ODLs by describing a formative, two-phase study with 15 pediatric oncology clinicians. In Phase I, we analyzed data from ethnographic interviews in a pediatric oncology setting to capture the needs of nurses, nurse practitioners, and oncologists. In Phase II, we constructed two low-fidelity dashboard display prototypes, populated with Visual ODLs contributed by actual adolescent oncology patients, and we subsequently interviewed pediatric oncology clinicians who reviewed each dashboard design. Findings from our study contribute four key design objectives for presenting interactive Visual ODL dashboards in pediatric oncology, along with three use cases for using these dashboards for symptom tracking and communication.

Visual ODLs: Co-Designing Patient-Generated Observations of Daily Living to Support Data-Driven Conversations in Pediatric Care.
Teens with complex chronic illnesses have difficulty understanding and articulating symptoms such as pain and emotional distress. Yet, symptom communication plays a central role in clinical care and illness management. To understand how design can help overcome these challenges, we created a visual library of 72 sketched illustrations, informed by the Observations of Daily Living framework along with insights from 11 clinician interviews. We utilized our library with storyboarding techniques, free-form sketching, and interviews, in co-design sessions with 13 pairs of chronically-ill teens and their parents. We found that teens depicted symptoms as being interwoven with narratives of personal and social identity. Teens and parents were enthusiastic about collaboratively-generated, interactive storyboards as a tracking and communication mechanism, and suggested three ways in which they could aid in communication and coordination with informal and formal caregivers. In this paper, we detail these findings, to guide the design of tools for symptom-tracking and incorporation of patient-generated data into pediatric care.

Parental control applications are designed to help parents monitor their teens and protect them from online risks. Generally, parents are considered the primary stakeholders for these apps; therefore, the apps often emphasize increased parental control through restriction and monitoring. By taking a developmental perspective and a Value Sensitive Design approach, we explore the possibility of designing more youth-centric online safety features. We asked 39 undergraduate students in the United States to create design charrettes of parental control apps that would better represent teens as stakeholders. As emerging adults, students discussed the value tensions between teens and parents and designed features to reduce and balance these tensions. While they emphasized safety, the students also designed to improve parent-teen communication, teen autonomy and privacy, and parental support. Our research contributes to the adolescent online safety literature by presenting design ideas from emerging adults that depart from the traditional paradigm of parental control. We also make a pedagogical contribution by leveraging design charrettes as a classroom tool for engaging college students in the design of youth-centered apps. We discuss why features that support parent-teen cooperation, teen privacy, and autonomy may be more developmentally appropriate for adolescents than existing parental control app designs.more » « less

We conducted an exploratory interview study with 10 undergraduate college students (ages 18-21) to get their feedback on how to best design a research study that asks teens (ages 13-17) to share portions of their Instagram data with their parents and discuss their online risk experiences. These young adults felt that teens should have as much control as possible when sharing their data, including the way that it was used in discussions with their parents. Our findings highlight the need to ensure researchers preserve the privacy and confidentiality of teens’ social media data.more » « less

null (Ed.)Traditional parental control applications designed to protect children and teens from online risks do so through parental restrictions and privacy-invasive monitoring. We propose a new approach to adolescent online safety that aims to strike a balance between a teen’s privacy and their online safety through active communication and fostering trust between parents and children. We designed and developed an Android “app” called Circle of Trust and conducted a mixed methods user study of 17 parent-child pairs to understand their perceptions about the app. Using a within-subjects experimental design, we found that parents and children significantly preferred our new app design over existing parental control apps in terms of perceived usefulness, ease of use, and behavioral intent to use. By applying a lens of Value Sensitive Design to our interview data, we uncovered that parents and children who valued privacy, trust, freedom, and balance of power preferred our app over traditional apps. However, those who valued transparency and control preferred the status quo. Overall, we found that our app was better suited for teens than for younger children.more » « less

Our research aims to highlight and alleviate the complex tensions around online safety, privacy, and smartphone usage in families so that parents and teens can work together to better manage mobile privacy and security-related risks. We developed a mobile application ("app") for Community Oversight of Privacy and Security ("CO-oPS") and had parents and teens assess whether it would be applicable for use with their families. CO-oPS is an Android app that allows a group of users to co-monitor the apps installed on one another's devices and the privacy permissions granted to those apps. We conducted a study with 19 parent-teen (ages 13-17) pairs to understand how they currently managed mobile safety and app privacy within their family and then had them install, use, and evaluate the CO-oPS app. We found that both parents and teens gave little consideration to online safety and privacy before installing new apps or granting privacy permissions. When using CO-oPS, participants liked how the app increased transparency into one another's devices in a way that facilitated communication, but were less inclined to use features for in-app messaging or to hide apps from one another. Key themes related to power imbalances between parents and teens surfaced that made co-management challenging. Parents were more open to collaborative oversight than teens, who felt that it was not their place to monitor their parents, even though both often believed parents lacked the technological expertise to monitor themselves. Our study sheds light on why collaborative practices for managing online safety and privacy within families may be beneficial but also quite difficult to implement in practice. We provide recommendations for overcoming these challenges based on the insights gained from our study.more » « less
