Title: Investigating post-earthquake hospital transportation for casualties through agent-based modeling
We develop a methodology for modeling post-earthquake hospital needs and accessibility, coupling a probabilistic regional casualty risk model with a novel agent-based human behavior model. We use it to study hospital transportation in two socio-economically distinct districts of Lima, Peru, after a hypothetical moment magnitude 8.0 event. Our work provides insights into the infrastructure and socio-economic vulnerability factors that determine timely healthcare access, across various hypothetical behavioral and policy contexts/scenarios. In the baseline scenario that assumes households respond to the emergency independently (i.e. transport only their own members to hospital) but there is coordination among hospitals (i.e. ambulance sharing), car ownership is key for rapid healthcare access. Twenty-four percent of non-life-threatening casualties in the district with 57% car ownership will arrive at hospital in the first 45 min; only 6% of such patients in the more economically deprived district (18% car ownership) will reach hospital in that time. If car-owning neighbors help transporting patients without cars, over 96% of non-life-threatening casualties will reach hospital in the first 45 min in both districts, significantly reducing intra-regional disparities in healthcare access. Arrival times would nearly halve compared to the baseline scenario if the number of available ambulances doubled. If hospitals do not share ambulances, arrival times of non-life-threatening and threatening casualties show a notable reduction in one district (by 56% and 42%, respectively) but a significant increase in the other, more economically deprived district (by 109% and 97%, respectively), compared to the baseline case. The arrival times presented in this study are specific to the studied context, and the appropriate assumptions made. We benchmark the performance of the methodology using patient arrival times from the 2023 Turkey-Syria earthquake, yielding an acceptable outcome (i.e. similar temporal trends in the proportion of hospital arrivals).   1. Healthcare capacity shortage contributes to poor access in many countries. Moreover, rapid urbanization often occurring in these countries has exacerbated the imbalance between healthcare capacity and need. One way to address the above challenge is expanding the total capacity and redistributing the capacity spatially. In this research, we studied the problem of locating new hospitals in a two-tier outpatient care system comprising multiple central and district hospitals, and upgrading existing district hospitals to central hospitals. We formulated the problem with a discrete location optimization model. To parameterize the optimization model, we used a multinomial logit model to characterize individual patients’ diverse hospital choice and to quantify the patient arrival rates at each hospital accordingly. To solve the hard nonlinear combinatorial optimization problem, we developed a queueing network model to approximate the impact of hospital locations on patient flows. We then proposed a multi-fidelity optimization approach, which involves both the aforementioned queuing network model as a surrogate and a self-developed stochastic simulation as the high-fidelity model. With a real-world case study of Shanghai, we demonstrated the changes in the care network and examined the impacts on the network design by population center emergence, governmental budget change and considering patients with different age groups or income levels. Note to Practitioners —Our work focuses on improving system-wide care access in a two-tier care network. We believe that our work can lead to effective development of a location analytics tool for city-wide healthcare system planners. We also think the importance of this study is further strengthened by the case studies based on real-world hospital choice experimental data from Shanghai, China, a region suffering from the imbalance between healthcare capacity and need. Our case studies are expected to make recommendations on care facility expansion and dispersion to better align with the spatial distribution of residential communities and patient hospital choice behavior.

Abstract Objective: The aim of this study was to investigate the performance of key hospital units associated with emergency care of both routine emergency and pandemic (COVID-19) patients under capacity enhancing strategies. Methods: This investigation was conducted using whole-hospital, resource-constrained, patient-based, stochastic, discrete-event, simulation models of a generic 200-bed urban U.S. tertiary hospital serving routine emergency and COVID-19 patients. Systematically designed numerical experiments were conducted to provide generalizable insights into how hospital functionality may be affected by the care of COVID-19 pandemic patients along specially designated care paths, under changing pandemic situations, from getting ready to turning all of its resources to pandemic care. Results: Several insights are presented. For example, each day of reduction in average ICU length of stay increases intensive care unit patient throughput by up to 24% for high COVID-19 daily patient arrival levels. The potential of 5 specific interventions and 2 critical shifts in care strategies to significantly increase hospital capacity is also described. Conclusions: These estimates enable hospitals to repurpose space, modify operations, implement crisis standards of care, collaborate with other health care facilities, or request external support, thereby increasing the likelihood that arriving patients will find an open staffed bed when 1 is needed.

  3. The world has experienced an unprecedented global health crisis since 2020, the COVID-19 pandemic, which inflicted massive burdens on countries' healthcare systems. During the peaks of the pandemic, the shortages of intensive care unit (ICU) beds illustrated a critical vulnerability in the fight. Many individuals suffering the effects of COVID-19 had difficulty accessing ICU beds due to insufficient capacity. Unfortunately, it has been observed that many hospitals do not have enough ICU beds, and the ones with ICU capacity might not be accessible to all population strata. To remedy this going forward, field hospitals could be established to provide additional capacity in helping emergency health situations such as pandemics; however, location selection is a crucial decision ultimately for this purpose. As such, we consider finding new field hospital locations to serve the demand within certain travel-time thresholds, while accounting for the presence of vulnerable populations. A multi-objective mathematical model is proposed in this paper that maximizes the minimum accessibility and minimizes the travel time by integrating the Enhanced 2-Step Floating Catchment Area (E2SFCA) method and travel-time-constrained capacitated p-median model. This is performed to decide on the locations of field hospitals, while a sensitivity analysis addresses hospital capacity, demand level, and the number of field hospital locations. Four counties in Florida are selected to implement the proposed approach. Findings can be used to identify the ideal location(s) of capacity expansions concerning the fair distribution of field hospitals in terms of accessibility with a specific focus on vulnerable strata of the population

Based in a research-practice partnership around district-wide computational thinking (CT) Pathways, this paper explores how six districts utilized the CT Engagement Inventory to examine if and how students are engaged in computing learning opportunities and write inclusive CT pathway goals. We found the CT Engagement Inventory supported districts in articulating inclusive pathway goals that moved beyond focusing only on access and participation. Instead, goals focused on building capacity to make broader access and participation possible and examining the nature of student participation. This paper demonstrates a tool to support districts in ensuring inclusive computing learning opportunities reach all students.

The development of the transport network affects the socio-economic development of the territory and it is one of the most important factors in the growth of the level and quality of the population life. There is the need for a study of transport accessibility. In our work, we presented the mapping and assessment of changes in transport accessibility after the construction of service road. After the construction of the ESPO pipeline, a service road was built along it to maintenance the pipeline, which is located close to the district centers and crosses the local roads. This new road connected them into one network with year-round traffic. The object of our research is the Area of Oil and Gas Extraction in the Republic of Sakha (Yakutia) and the North Irkutsk region.We have created transport accessibility maps with and without all service roads, separately for winter and summer seasons. We have created maps for several district centers. We calculated transport accessibility using the method of constructing isochrones — lines of equal travel time to overcome the space relative to given points, using open GIS GRASS GIS. After construction, the company owner of this road gives permission to the municipal and federal services and local population use for free, but a preliminary application is required. There is a payment requirement and compliance with restrictions for transportation of commercial goods. After the construction of the ESPO pipeline, people who live close to the pipeline can reach to the district centers and neighboring districts by car year-round theoretically. The materials of this study can be useful in calculating the travel time on these roads, and finding priority areas for the construction of new roads.
