Title: Wearable‐Derived Patterns of Performance Fatigability During Gait in Spinal Muscular Atrophy
Introduction/aims: Despite disease-modifying therapies, fatigability persists in spinal muscular atrophy (SMA). Performance fatigability (PF) during the six-minute walk test (6MWT) is mostly unchanged in treated SMA. This cross-sectional study characterized PF using instrumented insoles. Methods: Ambulatory individuals with SMA (n = 14) and controls (HC) (n = 10) were included. Spatiotemporal and kinetic parameters were collected with custom-engineered instrumented insoles during the 6MWT. Linear mixed models analyzed parameter trends, with trend slope representing PF. Changes in mean velocity (Vavg) and stride-by-stride parameters were compared between minute 1 and 6 and between groups. Results: Decreases in Vavg were greatest for severe SMA (p < 0.001). Changes were found in stride length (SL) (p = 0.048) and stride velocity (SV) (p = 0.030) for severe SMA, and in stance phase (%St) (p = 0.012) and percent terminal double support (%DS) (p = 0.02) for mild SMA. SMA subgroups showed downward trends from minute 1 to 6 in SL, SV, and anterior-posterior center of pressure (AP-COP) (p < 0.001), and increases in Absolute COP-Cyclogram Asymmetry Index (|ASI|) (p < 0.05). Trends differed between severe SMA and other groups for SL, SV, %St, and %DS (p < 0.001), and for AP-COP and |ASI| (p < 0.05). Trends for SL (p < 0.001), SV and AP-COP (p < 0.01) differed between HC and mild SMA. Discussion: PF in SMA manifests as changes in gait parameters. Instrumented insoles revealed fatigue-related changes not captured with the conventional method of comparing the first and last minutes of the 6MWT. Spatiotemporal and kinetic parameters contribute to understanding of impairments and inform therapeutic development. Trial registration: ClinicalTrials.gov: NCT04193085.  more » « less

  1. Hydrothermal conversion (HC) is a promising thermochemical technology to produce biofuels and bioproducts from biomass. However, the disposal of HC aqueous product (HC-AP) is one of the biggest challenges. This research investigated a new biological method using a marine protist, Thraustochytrium striatum to treat HC-AP from hydrothermal carbonization of municipal solid waste. A full factorial experiment was carried out to examine the effects of operation conditions on HC-AP treatment regarding cell growth, cellular lipid accumulation, and removals of total organic carbon (TOC)/total nitrogen (TN)/total phosphorus (TP)/recalcitrant compounds. Among four factors, only dilution rate and nitrogen concentration had significant effects on all responses, while salinity and pH were insignificant. Under the optimum conditions, T. striatum achieved 1.27 g/L dry cell mass and 14 % cellular lipid content while removing 82 % TOC, 53 % TN, 94 % TP, and ∼89 % refractory compounds. This research offers a new biological platform for HC-AP treatment and valorization.

Recycling underutilized resources from food waste (FW) to agriculture through hydrothermal carbonization (HTC) has been proposed to promote a circular economy (CE) in food-energy-water (FEW) nexus. However, most HTC studies on FW were conducted at laboratory scale, and little is known on the efficacy and feasibility of field application of HTC products from FW, i.e. the aqueous phrase (AP) and solid hydrochar (HC), to support agriculture production. An integrated pilot-scale HTC system was established to investigate practical HTC reaction conditions treating FW. A peak temperature of 180 ◦C at a residence time of 60 min with 3 times AP recirculation were recommended as optimal HTC conditions to achieve efficient recovery of nutrients, and desirable AP and HC properties for agriculture application. Dilution of the raw AP and composting of the fresh HC are necessary as post-treatments to eliminate potential phytotoxicity. Applying properly diluted AP and the composted HC significantly improved plant growth and nutrient availability in both greenhouse and field trials, which were comparable to commercial chemical fertilizer and soil amendment. The HTC of FW followed with agricultural application of the products yielded net negative carbon emission of 􀀀 0.28 t CO2e t􀀀 1, which was much lower than the other alternatives of FW treatments. Economic profit could be potentially achieved by valorization of the AP as liquid fertilizer and HC as soil amendment. Our study provides solid evidences demonstrating the technical and economic feasibility of recycling FW to agriculture through HTC as a promising CE strategy to sustain the FEW nexus.

  4. Background:Sleep disturbances are associated with adverse perinatal outcomes. Thus, it is necessary to understand the continuous patterns of sleep during pregnancy and how moderators such as maternal age and pre-pregnancy body mass index impact sleep. Objective:This study aimed to examine the continuous changes in sleep parameters objectively (i.e. sleep stages, total sleep time, and awake time) in pregnant women and to describe the impact of maternal age and/or pre-pregnancy body mass index as moderators of these objective sleep parameters. Design:This was a longitudinal observational design. Methods:Seventeen women with a singleton pregnancy participated in this study. Mixed model repeated measures were used to describe weekly patterns, while aggregated changes describe these three pregnancy periods (10–19, 20–29, and 30–39 gestational weeks). Results:For the weekly patterns, we found significantly decreased deep (1.26 ± 0.18 min/week, p < 0.001), light (0.72 ± 0.37 min/week, p = 0.05), and total sleep time (1.56 ± 0.47 min/week, p < 0.001) as well as increased awake time (1.32 ± 0.34 min/week, p < 0.001). For the aggregated changes, we found similar patterns to weekly changes. Women (⩾30 years) had an even greater decrease in deep sleep (1.50 ± 0.22 min/week, p < 0.001) than those younger (0.84 ± 0.29 min/week, p = 0.04). Women who were both overweight/obese and ⩾30 years experienced an increase in rapid eye movement sleep (0.84 ± 0.31 min/week, p = 0.008), but those of normal weight (<30 years) did not. Conclusion:This study appears to be the first to describe continuous changes in sleep parameters during pregnancy at home. Our study provides preliminary evidence that sleep parameters could be potential non-invasive physiological markers predicting perinatal outcomes.

  5. Background/Objectives: To investigate macular vascular biomarkers for the detection of primary open-angle glaucoma (POAG). Methods: A total of 56 POAG patients and 94 non-glaucomatous controls underwent optical coherence tomography angiography (OCTA) assessment of macular vessel density (VD) in the superficial (SCP), and deep (DCP) capillary plexus, foveal avascular zone (FAZ) area, perimeter, VD, choriocapillaris and outer retina flow area. POAG patients were classified for severity based on the Glaucoma Staging System 2 of Brusini. ANCOVA comparisons adjusted for age, sex, race, hypertension, diabetes, and areas under the receiver operating characteristic curves (AUCs) for POAG/control differentiation were compared using the DeLong method. Results: Global, hemispheric, and quadrant SCP VD was significantly lower in POAG patients in the whole image, parafovea, and perifovea (p < 0.001). No significant differences were found between POAG and controls for DCP VD, FAZ parameters, and the retinal and choriocapillaris flow area (p > 0.05). SCP VD in the whole image and perifovea were significantly lower in POAG patients in stage 2 than stage 0 (p < 0.001). The AUCs of SCP VD in the whole image (0.86) and perifovea (0.84) were significantly higher than the AUCs of all DCP VD (p < 0.05), FAZ parameters (p < 0.001), and retinal (p < 0.001) and choriocapillaris flow areas (p < 0.05). Whole image SCP VD was similar to the AUC of the global retinal nerve fiber layer (RNFL) (AUC = 0.89, p = 0.53) and ganglion cell complex (GCC) thickness (AUC = 0.83, p = 0.42). Conclusions: SCP VD is lower with increasing functional damage in POAG patients. The AUC for SCP VD was similar to RNFL and GCC using clinical diagnosis as the reference standard.
