Background: We investigated the association between reproductive risk factors and breast cancer subtype in Black women. On the basis of the previous literature, we hypothesized that the relative prevalence of specific breast cancer subtypes might differ according to reproductive factors. Methods: We conducted a pooled analysis of 2,188 (591 premenopausal, 1,597 postmenopausal) Black women with a primary diagnosis of breast cancer from four studies in the southeastern United States. Breast cancers were classified by clinical subtype. Case-only polytomous logistic regression models were used to estimate ORs and 95% confidence intervals (CI) for HER2+ and triple-negative breast cancer (TNBC) status in relation to estrogen receptor–positive (ER+)/HER2− status (referent) for reproductive risk factors. Results: Relative to women who had ER+/HER2− tumors, women who were age 19–24 years at first birth (OR, 1.78; 95% CI, 1.22–2.59) were more likely to have TNBC. Parous women were less likely to be diagnosed with HER2+ breast cancer and more likely to be diagnosed with TNBC relative to ER+/HER2− breast cancer. Postmenopausal parous women who breastfed were less likely to have TNBC [OR, 0.65 (95% CI, 0.43–0.99)]. Conclusions: This large pooled study of Black women with breast cancer revealed etiologic heterogeneity among breast cancer subtypes. Impact: Black parous women who do not breastfeed are more likely to be diagnosed with TNBC, which has a worse prognosis, than with ER+/HER2− breast cancer.

Pilot Clinical Study Investigating the Thermal Physiology of Breast Cancer via High-Resolution Infrared Imaging
This descriptive study investigates breast thermal characteristics in females histologically diagnosed with unilateral breast cancer and in their contralateral normal breasts. The multi-institutional clinical pilot study was reviewed and approved by the Institutional Review Boards (IRBs) at participating institutions. Eleven female subjects with radiologic breast abnormalities were enrolled in the study between June 2019 and September 2019 after informed consent was obtained. Static infrared images were recorded for each subject. The Wilcoxon signed rank test was used to conduct paired comparisons in temperature data between breasts among the eight histologically diagnosed breast cancer subjects (n = 8). Localized temperatures of cancerous breast lesions were significantly warmer than corresponding regions in contralateral breasts (34.0 ± 0.9 °C vs. 33.2 ± 0.5 °C, p = 0.0142, 95% CI 0.25–1.5 °C). Generalized temperatures over cancerous breasts, in contrast, were not significantly warmer than corresponding regions in contralateral breasts (33.9 ± 0.8 °C vs. 33.4 ± 0.4 °C, p = 0.0625, 95% CI −0.05–1.45 °C). Among the breast cancers enrolled, breast cancers elevated temperatures locally at the site of the lesion (localized hyperthermia), but not over the entire breast (generalized hyperthermia).

ImportanceScreening with low-dose computed tomography (CT) has been shown to reduce mortality from lung cancer in randomized clinical trials in which the rate of adherence to follow-up recommendations was over 90%; however, adherence to Lung Computed Tomography Screening Reporting & Data System (Lung-RADS) recommendations has been low in practice. Identifying patients who are at risk of being nonadherent to screening recommendations may enable personalized outreach to improve overall screening adherence. ObjectiveTo identify factors associated with patient nonadherence to Lung-RADS recommendations across multiple screening time points. Design, Setting, and ParticipantsThis cohort study was conducted at a single US academic medical center across 10 geographically distributed sites where lung cancer screening is offered. The study enrolled individuals who underwent low-dose CT screening for lung cancer between July 31, 2013, and November 30, 2021. ExposuresLow-dose CT screening for lung cancer. Main Outcomes and MeasuresThe main outcome was nonadherence to follow-up recommendations for lung cancer screening, defined as failing to complete a recommended or more invasive follow-up examination (ie, diagnostic dose CT, positron emission tomography–CT, or tissue sampling vs low-dose CT) within 15 months (Lung-RADS score, 1 or 2), 9 months (Lung-RADS score, 3), 5 months (Lung-RADS score, 4A), or 3 months (Lung-RADS score, 4B/X). Multivariable logistic regression was used to identify factors associated with patient nonadherence to baseline Lung-RADS recommendations. A generalized estimating equations model was used to assess whether the pattern of longitudinal Lung-RADS scores was associated with patient nonadherence over time. ResultsAmong 1979 included patients, 1111 (56.1%) were aged 65 years or older at baseline screening (mean [SD] age, 65.3 [6.6] years), and 1176 (59.4%) were male. The odds of being nonadherent were lower among patients with a baseline Lung-RADS score of 1 or 2 vs 3 (adjusted odds ratio [AOR], 0.35; 95% CI, 0.25-0.50), 4A (AOR, 0.21; 95% CI, 0.13-0.33), or 4B/X, (AOR, 0.10; 95% CI, 0.05-0.19); with a postgraduate vs college degree (AOR, 0.70; 95% CI, 0.53-0.92); with a family history of lung cancer vs no family history (AOR, 0.74; 95% CI, 0.59-0.93); with a high age-adjusted Charlson Comorbidity Index score (≥4) vs a low score (0 or 1) (AOR, 0.67; 95% CI, 0.46-0.98); in the high vs low income category (AOR, 0.79; 95% CI, 0.65-0.98); and referred by physicians from pulmonary or thoracic-related departments vs another department (AOR, 0.56; 95% CI, 0.44-0.73). Among 830 eligible patients who had completed at least 2 screening examinations, the adjusted odds of being nonadherent to Lung-RADS recommendations at the following screening were increased in patients with consecutive Lung-RADS scores of 1 to 2 (AOR, 1.38; 95% CI, 1.12-1.69). Conclusions and RelevanceIn this retrospective cohort study, patients with consecutive negative lung cancer screening results were more likely to be nonadherent with follow-up recommendations. Abstract BackgroundBreast cancer poses a significant health risk to women worldwide, with approximately 30% being diagnosed annually in the United States. The identification of cancerous mammary tissues from non-cancerous ones during surgery is crucial for the complete removal of tumors. ResultsOur study innovatively utilized machine learning techniques (Random Forest (RF), Support Vector Machine (SVM), and Convolutional Neural Network (CNN)) alongside Raman spectroscopy to streamline and hasten the differentiation of normal and late-stage cancerous mammary tissues in mice. The classification accuracy rates achieved by these models were 94.47% for RF, 96.76% for SVM, and 97.58% for CNN, respectively. To our best knowledge, this study was the first effort in comparing the effectiveness of these three machine-learning techniques in classifying breast cancer tissues based on their Raman spectra. Moreover, we innovatively identified specific spectral peaks that contribute to the molecular characteristics of the murine cancerous and non-cancerous tissues. ConclusionsConsequently, our integrated approach of machine learning and Raman spectroscopy presents a non-invasive, swift diagnostic tool for breast cancer, offering promising applications in intraoperative settings.more » « less

Infrared breast thermography has been associated with the early detection of breast cancer. However, findings in previous studies have been inconclusive. The upright position of subjects during imaging introduces errors in interpretation because it blocks the optical access in the inframammary fold region and alters the temperature due to contact between breast and chest wall. These errors can be avoided by imaging breasts in prone position. Although the numerical simulations provide insight into thermal characteristics of the female breast with a tumor, most simulations in the past have used cubical and hemispherical breast models. We hypothesize that a breast model with the actual breast shape will provide true thermal characteristics that are useful in tumor detection. A digital breast model in prone position is developed to generate the surface temperature profiles for breasts with tumors. The digital breast model is generated from sequential MRI images and simulations are performed using Finite Volume Method employing Pennes bioheat equation. We investigated the effect of varying the tumor metabolic activity on the surface temperature profile. We compared the surface temperature profile for various tumor metabolic activities with a case without tumor. The resulting surface temperature rise near the location of the tumor was between 0.665 and 1.023 °C, detectable using modern Infrared cameras. This is the first time that numerical simulations are conducted in a model with the actual breast shape in prone position to study the surface temperature changes induced by breast cancer.more » « less

Abstract Focal therapies such as hyperthermia have been successfully used to treat solid localized tumors; however, they are not easily applied to cancers that may present in a disseminated form such as ovarian cancer. To address this need, iron oxide (IO) particles were incorporated into microporous poly(caprolactone) scaffolds previously shown to recruit disseminating cancer cells. Under an alternating magnetic field, IO‐loaded scaffolds exhibited heating and killed ID8 ovarian cancer cells in vitro. After implantation in the intraperitoneal cavity of mice, IO‐loaded scaffolds became infiltrated with tissue after 6–7 weeks, and infiltrated cells were successfully treated ex vivo. Finally, IO‐loaded scaffolds noninvasively killed infiltrated cells in vivo as evidenced by decreases in number of nuclei. These studies demonstrate the promising use of IO‐loaded scaffolds as a tool for noninvasive hyperthermia, which could be an innovative modality for treatment of disseminated cancers.more » « less
