Title: Understanding the Last Mile Transportation Concept Impacting Underserved Global Communities to Save Lives During COVID-19 Pandemic
The underserved population could be at risk during the times of crisis, unless there is strong involvement from government agencies such as local and state Health departments and federal Center for Disease Control (CDC). The COVID-19 pandemic was a crisis of different proportion, creating a different type of burden on government agencies. Vulnerable communities including the elderly populations and communities of color have been especially hard hit by this pandemic. This forced these agencies to change their strategies and supply chains to support all populations receiving therapeutics. The National Science Foundation [National Science Foundation (NSF) Award Abstract # 2028612] funded RAID Labs to help federal agencies with strategies. This paper is based on a NSF funded grant to work on investigating supply chain strategies that would minimize the impact on underserved populations during pandemic. This NSF funded study identified the phenomena of last mile importance. The last mile transportation concept was critical in saving lives during the pandemic for underserved populations. The supply chain model then maximizes social goods by sending drugs or vaccines to the communities that need it the most regardless of ability to pay. The outcome of this study helped us prioritize the communities that need the vaccines the most. This informs our supply chain model to shift resources to these areas showing the value in real time prioritization of the COVID-19 supply chain. This paper provides information can be used in our healthcare supply chain model to ensure timely delivery of vaccines and supplies to COVID-19 patients that are the most vulnerable and hence the overall impact of COVID-19 can be minimized. The use of electrical vehicles for last mile transportation can help in significantly fighting the climate change.  more » « less

    During COVID-19, social media has played an important role for public health agencies and government stakeholders (i.e. actors) to disseminate information regarding situations, risks, and personal protective action inhibiting disease spread. However, there have been notable insufficient, incongruent, and inconsistent communications regarding the pandemic and its risks, which was especially salient at the early stages of the outbreak. Sufficiency, congruence and consistency in health risk communication have important implications for effective health safety instruction as well as critical content interpretability and recall. It also impacts individual- and community-level responses to information. This research employs text mining techniques and dynamic network analysis to investigate the actors’ risk and crisis communication on Twitter regarding message types, communication sufficiency, timeliness, congruence, consistency and coordination. We studied 13,598 pandemic-relevant tweets posted over January to April from 67 federal and state-level agencies and stakeholders in the U.S. The study annotates 16 categories of message types, analyzes their appearances and evolutions. The research then identifies inconsistencies and incongruencies on four critical topics and examines spatial disparities, timeliness, and sufficiency across actors and message types in communicating COVID-19. The network analysis also reveals increased communication coordination over time. The findings provide unprecedented insight of Twitter COVID-19 information dissemination which may help to inform public health agencies and governmental stakeholders future risk and crisis communication strategies related to global hazards in digital environments.

  2. The United States takes a federalist approach to pandemic responses while the bulk of pandemic powers sits at the state level. Thus, comprehensive accounts of how state health officials managed the crisis and how the federal government affected those efforts are needed to better understand the governmental response to the COVID-19 pandemic. This article reports the results of semistructured interviews with 29 state-level policy actors from 16 US states. Interviewees discussed multiple aspects of the US federal COVID-19 response that affected the response in their states, including communications with the public, intergovernmental communications, and federal actions regarding various aspects of health service preparedness including emergency funding, procurement, testing capacity, vaccine development and distribution, and data systems. This research enriches the discussion about US pandemic preparedness and response, and indicates that alignment of public communications across government levels, enhanced intergovernmental communication, inclusion of rural perspectives, and federal investment in and sustainment of health service preparedness are key factors that can improve future US pandemic responses.

    Background The COVID-19 health crisis has disproportionately impacted populations who have been historically marginalized in health care and public health, including low-income and racial and ethnic minority groups. Members of marginalized communities experience undue barriers to accessing health care through virtual care technologies, which have become the primary mode of ambulatory health care delivery during the COVID-19 pandemic. Insights generated during the COVID-19 pandemic can inform strategies to promote health equity in virtual care now and in the future. Objective The aim of this study is to generate insights arising from literature that was published in direct response to the widespread use of virtual care during the COVID-19 pandemic, and had a primary focus on providing recommendations for promoting health equity in the delivery of virtual care. Methods We conducted a narrative review of literature on health equity and virtual care during the COVID-19 pandemic published in 2020, describing strategies that have been proposed in the literature at three levels: (1) policy and government, (2) organizations and health systems, and (3) communities and patients. Results We highlight three strategies for promoting health equity through virtual care that have been underaddressed in this literature: (1) simplifying complex interfaces and workflows, (2) using supportive intermediaries, and (3) creating mechanisms through which marginalized community members can provide immediate input into the planning and delivery of virtual care. Conclusions We conclude by outlining three areas of work that are required to ensure that virtual care is employed in ways that are equity enhancing in a post–COVID-19 reality.

    Increased demand for medical supplies, and specifically respirators and face masks, during the Covid-19 pandemic along with the inability of legitimate suppliers to meet these needs created a window of opportunity for counterfeiters to capitalize on the supply chain disruptions caused by a global health crisis. Both legitimate and illicit businesses began shifting their scope from sectors such as textiles to producing and distributing personal protective equipment (PPE), many of which were counterfeit or unauthentic products and thus unable to properly protect users. To study cost-effective disruption strategies, this study proposes a simulation-optimization framework. The framework is used to model counterfeiters’ behavior and analyze the effectiveness of different disruption strategies for counterfeit PPE supply chains during the Covid-19 pandemic.

  5. Social media can be a significant tool for transportation and transit agencies providing passengers with real-time information on traffic events. Moreover, COVID-19 and other limitations have compelled the agencies to engage with travelers online to promote public knowledge about COVID-related issues. It is, therefore, important to understand the agencies’ communication patterns. In this original study, the Twitter communication patterns of different transportation actors—types of message, communication sufficiency, consistency, and coordination—were examined using a social media data-driven approach applying text mining techniques and dynamic network analysis. A total of 850,000 tweets from 395 different transportation and transit agencies, starting in 2018 and the periods before, during and after the pandemic, were studied. Transportation agencies (federal, state, and city) were found to be less active on Twitter and mostly discussed safety measures, project management, and so forth. By contrast, the transit agencies (local bus and light, heavy, and commuter rail) were more active on Twitter and shared information about crashes, schedule information, passenger services, and so forth. Moreover, transportation agencies shared minimal pandemic safety information than transit agencies. Dynamic network analysis reveals interaction patterns among different transportation actors that are poorly connected and coordinated among themselves and with different health agencies (e.g., Centers for Disease Control and Prevention [CDC] and the Federal Emergency Management Agency [FEMA]). The outcome of this study provides understanding to improve existing communication plans, critical information dissemination efficacy, and the coordination of different transportation actors in general and during unprecedented health crises.
